Erybac 500 mg tablet is an antibiotic containing Erythromycin, belonging to the macrolide class. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
The usual dose is 500 mg every 12 hours, or 250 mg every 6 hours, for 7-14 days depending on the severity and type of infection. Some severe infections may require higher doses.
Elderly
Similar to adult dosage; however, caution should be exercised in elderly patients with hepatic impairment.
Renal_impairment
No dose adjustment is generally necessary for mild to moderate renal impairment. For severe renal impairment (creatinine clearance <10 mL/min), a dose reduction or increased dosing interval may be considered (e.g., 250-500 mg every 18-24 hours).
How to Take
Erybac tablets should be swallowed whole with water. It can be taken with or without food, but taking it with a small amount of food may help reduce gastrointestinal upset. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Erythromycin works by inhibiting bacterial protein synthesis. It binds to the 50S ribosomal subunit of susceptible bacteria, thereby blocking the translocation of peptidyl-tRNA and preventing the growth and replication of bacteria (bacteriostatic effect).

Excretion
Primarily excreted in the bile and feces. A small percentage (2-5%) is excreted unchanged in the urine.

Side Effects
Contraindications
- •Hypersensitivity to erythromycin or any macrolide antibiotic.
- •Concomitant use with astemizole, cisapride, terfenadine, or pimozide due to risk of life-threatening cardiac arrhythmias.
- •Concomitant use with ergotamine or dihydroergotamine due to risk of acute ergot toxicity.
Drug Interactions
Digoxin
Increased serum digoxin levels, leading to potential toxicity.
Warfarin
Increased anticoagulant effect, leading to increased bleeding risk.
Cimetidine
May increase erythromycin levels.
Statins (e.g., Simvastatin, Lovastatin)
Increased risk of myopathy and rhabdomyolysis due to inhibition of statin metabolism.
Antacids containing aluminum or magnesium
May reduce erythromycin absorption.
Carbamazepine, Theophylline, Cyclosporine
Increased serum levels of these drugs, leading to potential toxicity.
Storage

Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and abdominal pain. Hearing loss (reversible) may occur. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered in acute overdose.
Pregnancy & Lactation
Pregnancy Category B. Erythromycin is generally considered safe for use during pregnancy, though some studies suggest a small increased risk of cardiovascular malformations with first-trimester use (data are conflicting). It is excreted in breast milk in small amounts but is generally considered compatible with breastfeeding; however, monitor infants for gastrointestinal disturbances (e.g., diarrhea) and fungal infections.
