Description
Overview of the medicine
Eryzen 125 mg Suspension is an antibiotic used to treat a wide variety of bacterial infections. It belongs to the macrolide group of antibiotics and works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250-500 mg every 6 hours, or 500-1000 mg every 12 hours, depending on severity of infection. Max 4g/day.
Elderly
No specific dose adjustment needed unless severe hepatic or renal impairment.
Children
30-50 mg/kg/day in divided doses (usually 4 doses), for severe infections up to 100 mg/kg/day. Eryzen 125 mg suspension: for children 2-8 years, typically 5 ml (125 mg) every 6 hours.
Renal_impairment
No dose adjustment generally required for mild to moderate renal impairment. For severe impairment (CrCl <10 mL/min), dose reduction may be considered.
How to Take
Take orally, preferably on an empty stomach (1 hour before or 2 hours after meals) for optimal absorption. May be taken with food to reduce gastrointestinal upset. Shake well before use.
Mechanism of Action
Erythromycin acts by binding to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis. This action is primarily bacteriostatic, but can be bactericidal at higher concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Within 1 hour (for antibacterial effect)
Excretion
Mainly through bile and feces, a small amount excreted in urine.
Half life
1.5-2 hours (in adults with normal renal function)
Absorption
Readily absorbed from the gastrointestinal tract, especially in the fasting state. Absorption may be variable and incomplete.
Metabolism
Primarily hepatic, via CYP3A4 system.
Side Effects
Contraindications
- •Hypersensitivity to erythromycin or any other macrolide antibiotics
- •Pre-existing liver disease
- •Concurrent use with terfenadine, astemizole, cisapride, pimozide (due to risk of QT prolongation and cardiac arrhythmias)
Drug Interactions
Digoxin
Increased digoxin levels, potentially leading to toxicity.
Warfarin
Increased anticoagulant effect, leading to increased risk of bleeding.
Theophylline
Increased theophylline levels, potentially leading to toxicity.
Statins (e.g., Simvastatin, Lovastatin)
Increased risk of myopathy/rhabdomyolysis due to inhibition of statin metabolism.
QT-prolonging drugs (e.g., Terfenadine, Astemizole, Cisapride, Pimozide)
Significantly increased risk of life-threatening cardiac arrhythmias (contraindicated).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and discard after 7-14 days.
Overdose
Symptoms of overdose include severe gastrointestinal upset (nausea, vomiting, diarrhea), hearing loss (reversible), and cardiac arrhythmias. Treatment is symptomatic and supportive, including gastric lavage or activated charcoal if recent ingestion.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy, but use only if clearly needed. Erythromycin is excreted in breast milk; caution is advised when administered to a nursing mother.
