Overview of the medicine
Erythromycin is a macrolide antibiotic used to treat a wide range of bacterial infections, including respiratory tract infections, skin infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
250-500 mg every 6-12 hours, depending on the severity of the infection and specific indication. Usually 250 mg four times daily or 500 mg twice daily.
Elderly
No specific dose adjustment based on age alone, but caution with renal or hepatic impairment.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. For severe impairment (CrCl <10 mL/min), dosage reduction may be considered.
How to Take
Orally, with a full glass of water. Can be taken with or without food, but some forms are better absorbed on an empty stomach.
Mechanism of Action
Binds to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis.

Side Effects
Contraindications
- •Hypersensitivity to erythromycin or any macrolide antibiotic
- •Co-administration with cisapride, pimozide, or ergot alkaloids due to risk of serious arrhythmias
- •History of cholestatic jaundice/hepatic dysfunction associated with prior erythromycin use
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased anticoagulant effect, leading to bleeding risk.
Carbamazepine
Increased carbamazepine levels, leading to toxicity.
Cisapride, Pimozide, Ergot alkaloids
Significant risk of QT prolongation and serious arrhythmias. Contraindicated.
Statins (e.g., simvastatin, lovastatin)
Increased risk of myopathy/rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe nausea, vomiting, abdominal pain, diarrhea, and reversible hearing loss. Treatment is symptomatic and supportive; gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy, but use only if clearly needed. Excreted in breast milk; caution advised, observe infant for GI disturbances.
