Description
Overview of the medicine
Esmax 20 mg capsule contains Esomeprazole Magnesium, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It treats conditions such as GERD, erosive esophagitis, and ulcers.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on the condition. For GERD, typically 20-40 mg once daily for 4-8 weeks. For H. pylori eradication, 20 mg twice daily for 7-10 days with antibiotics.
Elderly
No dosage adjustment generally required, but caution in severely impaired hepatic function.
Renal_impairment
No dosage adjustment is needed for patients with impaired renal function.
How to Take
Take orally, at least one hour before a meal. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby blocking the final step of acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Metabolites are primarily excreted in the urine (approx. 80%) and in the feces (approx. 20%).
Half life
Plasma half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% at 20 mg and increases to 89% with repeated dosing.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Co-administration with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Nelfinavir
Coadministration is contraindicated due to decreased nelfinavir plasma levels.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel. Use with caution or consider alternative.
Methotrexate
Increased methotrexate levels, consider temporary withdrawal of PPI.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal disturbances and fatigue. Treatment is symptomatic and supportive. Esomeprazole is highly protein-bound and not readily dialyzable.

Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Excreted in breast milk, use with caution.
