Description
Overview of the medicine
Esmax 20 mg tablet contains Esomeprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions like gastroesophageal reflux disease (GERD), stomach ulcers, and to prevent NSAID-induced ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori: 20 mg twice daily for 7-10 days (with antibiotics). For Zollinger-Ellison syndrome: Initial dose 40 mg twice daily.
Elderly
No specific dose adjustment is needed for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take one tablet orally at least one hour before a meal. Swallow the tablet whole; do not chew or crush it.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This irreversible binding prevents the final step of acid secretion, thereby reducing gastric acidity.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma levels occur within 1-2 hours. Bioavailability is about 64% after a single 40 mg dose and increases to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin exposure.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased and prolonged elevated serum methotrexate levels.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
