Description
Overview of the medicine
Esmax 40 mg capsule is a proton pump inhibitor (PPI) that reduces the amount of acid produced in your stomach. It is used to treat acid-related diseases of the stomach and gut, such as heartburn, acid reflux, and peptic ulcer disease.
Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Maintenance: 20 mg once daily. H. pylori eradication: 40 mg once daily with antibiotics for 10 days. Zollinger-Ellison Syndrome: Initial dose of 40 mg twice daily, adjusted as needed.
Elderly
No general dose adjustment is needed for the elderly; however, caution should be exercised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with mild to moderate renal impairment. For severe renal impairment, use with caution.
How to Take
Take the capsule orally, at least one hour before a meal. Swallow the capsule whole with water. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole works by irreversibly binding to the H+/K+-ATPase enzyme (the proton pump) on the secretory surface of the parietal cells in the stomach, thereby inhibiting gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression begins within 1 hour; full therapeutic effect typically observed within a few days.
Excretion
Approximately 80% of metabolites are excreted in the urine, and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration. Bioavailability increases from approximately 68% after a single dose to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any excipients.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Digoxin
Increased digoxin levels, monitor digoxin concentrations.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Reduced efficacy of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased methotrexate levels, especially with high-dose methotrexate.
Antifungals (e.g., Ketoconazole)
Reduced absorption due to increased gastric pH.
Storage
Store in a cool, dry place below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and reversible. Management involves symptomatic and supportive treatment. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Esomeprazole is excreted in human breast milk; use with caution in breastfeeding mothers.
