Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For duodenal ulcer: 20 mg once daily for 2-4 weeks. For H. pylori eradication: 20 mg twice daily for 7-14 days with antibiotics.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment, caution is advised.
How to Take
Take orally, usually once daily, at least one hour before a meal. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion. This leads to a reduction in gastric acid production.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Mainly through urine (approximately 80%) and feces (approximately 20%) as metabolites.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma levels occur within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole or any of its components
- •Hypersensitivity to other substituted benzimidazoles
- •Concomitant use with nelfinavir

Increased INR and bleeding risk

Increased tacrolimus levels
Clopidogrel
Reduced antiplatelet effect
Methotrexate
Increased plasma levels of methotrexate
Atazanavir/Nelfinavir
Reduced bioavailability of antiretrovirals
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, and GI disturbances. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug.
