Uses & Indications
Dosage
Adults
Not applicable for pediatric drop formulation; adult dosing is typically 20-40 mg daily.
Elderly
Not applicable for pediatric drop formulation.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Caution with severe impairment.
How to Take
Administer orally, usually 30-60 minutes before a meal. The drop can be mixed with a small amount of water, apple juice, or soft food (e.g., applesauce) immediately before administration. Do not chew or crush.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This irreversible inhibition blocks the final step in acid secretion, leading to a sustained reduction in gastric acid production.

Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Possible increased methotrexate levels, especially with high doses.
Atazanavir/Nelfinavir
Reduced plasma concentrations of atazanavir/nelfinavir.
Diazepam/Phenytoin/Warfarin
Increased plasma concentrations of these drugs (CYP2C19 substrates).
Storage

Overdose
Limited experience with overdose. Symptoms are generally mild and transient, including drowsiness, confusion, blurred vision, tachycardia, nausea, vomiting, sweating, headache, dry mouth. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnant women should only be when clearly needed. Esomeprazole is excreted in human breast milk; use with caution in breastfeeding mothers.
