Description
Overview of the medicine
Etoricoxib is a non-steroidal anti-inflammatory drug (NSAID) belonging to the coxib class, which selectively inhibits cyclooxygenase-2 (COX-2). It is used to relieve pain and inflammation in various conditions such as osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute gouty arthritis, and acute pain.
Uses & Indications
Dosage
Adults
For acute pain/primary dysmenorrhea/acute gouty arthritis: 120 mg once daily, for a maximum of 8 days for acute gout and for the shortest duration necessary for other acute conditions. For osteoarthritis: 30 mg or 60 mg once daily. For rheumatoid arthritis/ankylosing spondylitis: 90 mg once daily.
Elderly
No dosage adjustment generally required, but caution is advised due to increased risk of adverse effects in elderly patients.
Renal_impairment
Severe renal impairment (creatinine clearance <30 mL/min) is a contraindication. For moderate renal impairment (creatinine clearance 30-60 mL/min), the maximum recommended dose is 60 mg once daily.
How to Take
Take orally, with or without food. Food may slightly delay the onset of action but does not affect the overall absorption.
Mechanism of Action
Etoricoxib is a selective inhibitor of cyclooxygenase-2 (COX-2). COX-2 is responsible for the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. By inhibiting COX-2, etoricoxib reduces prostaglandin production, thereby alleviating pain and inflammation.

Onset
Pain relief may begin within 24 minutes for acute pain conditions.
Excretion
Approximately 70% is excreted in the urine and 20% in the feces, mainly as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations achieved in approximately 1 hour.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP) enzymes, particularly CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any component of the formulation
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic dysfunction
- •Severe renal impairment (creatinine clearance <30 mL/min)
- •Patients with congestive heart failure (NYHA II-IV)
- •Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Pregnancy and lactation
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Warfarin
Increased risk of bleeding, monitor INR.
Diuretics
Reduced diuretic and antihypertensive effects.
Rifampicin
May decrease etoricoxib plasma concentrations.
Methotrexate
Increased plasma methotrexate levels, potentially enhancing toxicity.
Oral Contraceptives
Increased plasma concentrations of ethinyl estradiol, potentially increasing adverse effects.
ACE Inhibitors / Angiotensin Receptor Blockers (ARBs)
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for etoricoxib. Hemodialysis is unlikely to be effective in removing etoricoxib.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester, due to potential fetal harm (e.g., premature closure of the ductus arteriosus). Avoid during breastfeeding as it may be excreted in breast milk.
