Description
Overview of the medicine
Etoricoxib is a selective cyclooxygenase-2 (COX-2) inhibitor, a type of non-steroidal anti-inflammatory drug (NSAID), used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gout.
Uses & Indications
Dosage
Adults
Rheumatoid arthritis: 90 mg once daily. Ankylosing spondylitis: 90 mg once daily. Acute pain (including post-operative dental pain): 90 mg once daily for maximum 5 days. Acute gouty arthritis: 120 mg once daily (for up to 8 days, physician discretion).
Elderly
No dose adjustment is generally necessary; however, caution is advised due to potential increased risk of adverse effects.
Renal_impairment
Mild to moderate renal impairment (creatinine clearance 30-80 mL/min): No dose adjustment. Severe renal impairment (creatinine clearance <30 mL/min): Not recommended.
How to Take
Oral administration. Can be taken with or without food. To be swallowed whole with water.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), which is responsible for the synthesis of prostaglandins, mediators of pain, inflammation, and fever. By inhibiting COX-2, etoricoxib reduces prostaglandin production, thereby alleviating pain and inflammation.
Pharmacokinetics
Onset
Within 24 minutes for acute pain (e.g., dental pain), within 1-2 hours for chronic conditions.
Excretion
Excreted primarily via urine (approximately 70%) and feces (approximately 30%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 100%. Peak plasma concentration is reached in approximately 1 hour.
Metabolism
Extensively metabolized by the liver, mainly by cytochrome P450 enzymes (predominantly CYP3A4), to form inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any component of the formulation
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic dysfunction
- •Severe renal impairment (CrCl <30 mL/min)
- •Inflammatory bowel disease
- •Congestive heart failure (NYHA Class II-IV)
- •Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Pregnancy and lactation
Drug Interactions
Lithium
May increase lithium plasma concentrations and toxicity.
Warfarin
May increase prothrombin time (INR); monitor closely.
Rifampicin
May decrease etoricoxib plasma concentrations.
Methotrexate
May increase plasma concentrations of methotrexate and its toxicity.
Diuretics/ACE Inhibitors/ARBs
May reduce the natriuretic effect of diuretics and diminish the antihypertensive effect of ACE inhibitors/ARBs; increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal disturbances, renal toxicity, and drowsiness. Management involves symptomatic and supportive treatment. There is no specific antidote.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Use in the third trimester of pregnancy may cause premature closure of the fetal ductus arteriosus. Not known if excreted in human milk, but likely to be, therefore advise against use.
