Description
Overview of the medicine
Famiclav 250 mg Tablet is a combination antibiotic used to treat a wide range of bacterial infections. It contains amoxicillin, a penicillin-type antibiotic, and clavulanic acid, a beta-lactamase inhibitor, which works by protecting amoxicillin from degradation by bacterial enzymes, thus extending its antibacterial spectrum.
Uses & Indications
Dosage
Adults
The usual adult dose is 250 mg/62.5 mg three times daily (every 8 hours) for 7-10 days, depending on the severity and type of infection. In severe infections, the dose may be increased to 500 mg/125 mg three times daily.
Elderly
No dosage adjustment is generally required in elderly patients unless there is severe renal impairment.
Renal_impairment
Dosage adjustments are necessary for patients with impaired renal function. For creatinine clearance between 10-30 mL/min, 250 mg/62.5 mg every 12 hours. For creatinine clearance less than 10 mL/min, 250 mg/62.5 mg every 24 hours. Hemodialysis patients should receive an additional dose during and after dialysis.
How to Take
Take the tablet whole with a glass of water, with or without food. Taking it at the start of a meal may reduce potential gastrointestinal discomfort. Do not chew or crush the tablet.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Clavulanic acid is a suicide inhibitor of beta-lactamase enzymes produced by some bacteria, which would otherwise inactivate amoxicillin. By inhibiting beta-lactamase, clavulanic acid restores and extends the antibacterial activity of amoxicillin against beta-lactamase-producing strains.
Pharmacokinetics
Onset
Antibacterial effects typically begin within 1 hour after administration.
Excretion
Approximately 50-70% of amoxicillin and 25-40% of clavulanic acid are excreted unchanged in the urine within the first 6 hours after administration. Both are eliminated primarily by renal excretion.
Half life
The elimination half-life is approximately 1-1.5 hours for amoxicillin and about 1 hour for clavulanic acid in adults with normal renal function.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration. Peak serum concentrations are generally reached within 1-2 hours.
Metabolism
Amoxicillin is partially metabolized to an inactive penicilloic acid. Clavulanic acid is extensively metabolized to inactive compounds.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, clavulanic acid, or any other penicillin antibiotic
- History of cholestatic jaundice or hepatic dysfunction associated with amoxicillin/clavulanate therapy
- Severe renal impairment (CrCl < 30 mL/min) without dose adjustment
Drug Interactions
Allopurinol
Increased incidence of rash when co-administered with amoxicillin.
Methotrexate
May increase methotrexate toxicity; monitor for adverse effects.
Oral contraceptives
May reduce the efficacy of oral contraceptives (though clinical significance is debated, advise additional barrier methods).
Warfarin and other oral anticoagulants
May prolong prothrombin time (INR); monitor closely.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. Rare cases of renal failure have been observed. Treatment is symptomatic and supportive. Amoxicillin/clavulanic acid can be removed from circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. While studies in animals have not shown harm, adequate and well-controlled studies in pregnant women are lacking. It should be used during pregnancy only if clearly needed. Both amoxicillin and clavulanic acid are excreted into breast milk; use with caution in breastfeeding mothers, as it may cause diarrhea or candidiasis in the infant.
