Dosage
Adults
The usual adult dose for Famiclav 500 mg Tablet is one 500 mg/125 mg tablet taken every 8 hours, or one 875 mg/125 mg tablet taken every 12 hours, depending on the severity and type of infection. Treatment duration is typically 7-10 days.
Elderly
No specific dose adjustment is required in elderly patients unless there is evidence of renal impairment.

Dose adjustments are necessary for patients with impaired renal function, based on creatinine clearance. Consult a physician for specific dosing recommendations.
How to Take
Take Famiclav 500 mg Tablet at the start of a meal to minimize potential gastrointestinal intolerance and to enhance absorption. Swallow the tablet whole with a glass of water. Do not crush, chew, or break the tablet.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Clavulanic acid is a beta-lactamase inhibitor that irreversibly binds to and inactivates beta-lactamase enzymes produced by bacteria, thus protecting amoxicillin from degradation and extending its antibacterial spectrum.

Excretion
Both amoxicillin and clavulanic acid are primarily eliminated via renal excretion (urine).
Half life
The elimination half-life for amoxicillin is approximately 1 to 1.5 hours, and for clavulanic acid, it is about 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration, with clavulanic acid enhancing the absorption of amoxicillin. Peak plasma concentrations are achieved within 1 to 2 hours.
Metabolism
Amoxicillin undergoes limited metabolism in the liver. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Patients with a history of severe hypersensitivity reaction (e.g., anaphylaxis) to amoxicillin, clavulanic acid, any penicillin, or other beta-lactam antibiotics.
- Patients with a history of amoxicillin/clavulanate-associated jaundice or hepatic dysfunction.
- Patients with infectious mononucleosis, as it may increase the risk of erythematous rash.
Drug Interactions
Probenecid
Reduces renal tubular secretion of amoxicillin, leading to increased and prolonged blood levels of amoxicillin.
Allopurinol
Co-administration may increase the incidence of skin rash, especially in patients with hyperuricemia.
Methotrexate
Amoxicillin may reduce the renal excretion of methotrexate, potentially increasing its toxicity. Concurrent use should be carefully monitored.
Oral Contraceptives
Amoxicillin may reduce the efficacy of oral contraceptives. Advise patients to use additional non-hormonal contraception.
Mycophenolate Mofetil
Reduced pre-dose concentration of the active metabolite of mycophenolate mofetil observed with co-administration.
Warfarin and other oral anticoagulants
May prolong prothrombin time and increase the risk of bleeding. Close monitoring of INR is recommended.

Overdose
In case of overdose, patients may experience gastrointestinal symptoms (e.g., abdominal pain, vomiting, diarrhea) and fluid and electrolyte imbalance. Crystalluria leading to renal failure has been observed. Treatment should be symptomatic and supportive. Gastric lavage may be performed, and adequate fluid intake should be maintained. Hemodialysis can effectively remove amoxicillin from the circulation.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed after consulting a doctor. Amoxicillin and clavulanic acid are excreted in breast milk; therefore, use with caution in nursing mothers and consult a physician.
