Description
Overview of the medicine
Feluric (Febuxostat) is a medication used to lower uric acid levels in the blood, primarily for the long-term management of chronic hyperuricemia in patients with gout.
Uses & Indications
Dosage
Adults
Initial dose of 40 mg once daily. If serum uric acid is >6 mg/dL after 2 weeks, increase to 80 mg once daily.
Elderly
No dose adjustment necessary.
Renal_impairment
No dose adjustment for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl <30 mL/min).
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Febuxostat works by selectively inhibiting xanthine oxidase, an enzyme responsible for converting purines into uric acid. By blocking this enzyme, Febuxostat reduces the production of uric acid in the body.
Pharmacokinetics
Onset
Uric acid lowering effect begins within 24 hours; full therapeutic effect typically within 2 weeks.
Excretion
Both renal (approximately 49%) and hepatic/fecal (approximately 45%) excretion.
Half life
Approximately 5-8 hours.
Absorption
Rapidly and well absorbed after oral administration, with peak plasma concentrations occurring within 1-1.5 hours.
Metabolism
Extensively metabolized in the liver via oxidation and subsequent glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to Febuxostat or any component of the formulation.
- •Concurrent treatment with azathioprine or mercaptopurine.
Drug Interactions
Theophylline
Febuxostat may increase plasma concentrations of theophylline. Caution is advised, and monitoring of theophylline levels may be necessary.
Azathioprine / Mercaptopurine
Febuxostat inhibits xanthine oxidase, which metabolizes azathioprine and mercaptopurine, leading to increased plasma concentrations and toxicity of these drugs. Co-administration is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Experience with overdose of Febuxostat is limited. In case of an overdose, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Pregnancy: There are no adequate and well-controlled studies in pregnant women. Use only if the potential benefit justifies the potential risk to the fetus. Lactation: It is not known whether Febuxostat is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
