Description
Overview of the medicine
Fenat 0.0025% eye drop contains Fenat, a prostaglandin analog, used to lower elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension. It effectively increases the outflow of aqueous humor from the eye.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) once daily in the evening. Do not exceed one drop daily as more frequent administration may decrease the IOP lowering effect.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
For ophthalmic use only. Instill one drop into the conjunctival sac of the affected eye(s). If using other ophthalmic medications, allow at least 5 minutes between applications.
Mechanism of Action
Fenat, a prostaglandin F2α analog, selectively binds to the prostaglandin F2α receptor, which is believed to increase the outflow of aqueous humor primarily through the uveoscleral pathway, thereby reducing intraocular pressure.
Pharmacokinetics
Onset
IOP reduction typically starts within 2-4 hours.
Excretion
Excreted mainly in the urine as inactive metabolites.
Half life
Plasma half-life of the active acid is very short (approximately 17 minutes) due to rapid systemic metabolism.
Absorption
Rapidly absorbed through the cornea, where it is hydrolyzed to its active acid form. Peak concentration in aqueous humor is reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver by beta-oxidation and ester hydrolysis to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to the active substance Fenat or to any of the excipients.
Drug Interactions
Other Prostaglandin Analogs
Concomitant use with other prostaglandin analogs is not recommended as it may lead to paradoxical increases in IOP.
Thimerosal-containing products
May lead to precipitation, thus separate administration is required.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Ocular overdose is unlikely to occur with topical administration. If an overdose occurs, treatment should be symptomatic. The eye can be rinsed with lukewarm water.
Pregnancy & Lactation
Pregnancy: Should not be used during pregnancy unless clearly necessary and the potential benefits outweigh the potential risks to the fetus. Lactation: It is unknown whether Fenat or its metabolites are excreted in human milk. Exercise caution when administering to a nursing mother.

Clinical Trials
Extensive clinical trials have demonstrated Fenat's efficacy in reducing IOP and its acceptable safety profile in patients with open-angle glaucoma and ocular hypertension. Studies have shown sustained IOP reduction over 24 hours with once-daily dosing.
Lab Monitoring
- Routine intraocular pressure (IOP) monitoring by an ophthalmologist.
- Regular eye examinations to check for any changes in iris color, eyelash growth, or other ocular effects.
Doctor Notes
- Counsel patients on the importance of consistent once-daily dosing, preferably in the evening, for optimal IOP control.
- Educate about potential ocular cosmetic changes (iris pigmentation, eyelash growth, periorbital darkening) and the need for regular follow-up.
- Advise patients using contact lenses to remove them prior to instillation and wait 15 minutes before reinsertion.
Patient Guidelines
- Remove contact lenses before instilling the drops and wait at least 15 minutes before reinserting them.
- Avoid allowing the tip of the dispensing container to touch the eye or any other surface to prevent contamination.
- If more than one topical ophthalmic drug is being used, they should be administered at least 5 minutes apart.
Missed Dose Advice
If a dose is missed, continue with the next dose as scheduled. Do not take a double dose to make up for a missed one.
Driving Precautions
Fenat 0.0025% eye drop may cause temporary blurred vision or other visual disturbances. If blurred vision occurs, wait until the vision clears before driving or operating machinery.
Lifestyle Advice
- Regular follow-up appointments with an ophthalmologist are crucial for managing glaucoma and monitoring treatment effectiveness.
- Maintain a healthy lifestyle, including a balanced diet and regular exercise, as part of overall eye health.
