Description
Overview of the medicine
Fenat (Fexofenadine Hydrochloride) is a non-sedating antihistamine primarily used to relieve symptoms associated with seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). While standard fexofenadine syrups are typically 30 mg/5 ml, this '1 mg' strength is highly unconventional for therapeutic dosing and may imply a specialized, very dilute formulation, or the need for a very large volume per dose.
Uses & Indications
Dosage
Adults
For Seasonal Allergic Rhinitis: 60 mg twice daily or 180 mg once daily. For Chronic Idiopathic Urticaria: 60 mg twice daily. (Note: A 1 mg syrup would require very large volumes for these therapeutic doses, e.g., 60-180 ml per dose if 1 mg/ml, or 300-900 ml if 1 mg/5 ml. Consult a healthcare professional for appropriate dosing with this specific strength.)
Elderly
No specific dosage adjustment recommended for elderly patients with normal renal function.
Renal_impairment
Adults with impaired renal function (creatinine clearance ≤ 10 mL/min): 60 mg once daily. (Note: Same volume considerations as above apply for the 1 mg syrup.)
Children_2_to_11_years
For Seasonal Allergic Rhinitis & Chronic Idiopathic Urticaria: 30 mg orally twice daily. (Note: This would be 30 ml if 1 mg/ml, or 150 ml if 1 mg/5 ml of the '1 mg' syrup. Consult a healthcare professional.)
Children_6_months_to_less_than_2_years
For Chronic Idiopathic Urticaria: 15 mg orally twice daily. (Note: This would be 15 ml if 1 mg/ml, or 75 ml if 1 mg/5 ml of the '1 mg' syrup. Consult a healthcare professional.)
How to Take
Administer orally. Shake the bottle well before use. May be taken with or without food, but avoid taking with fruit juices.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It blocks the action of histamine, a natural substance in the body that causes allergic symptoms, without significant central nervous system (CNS) penetration, thus causing less drowsiness.
Pharmacokinetics
Onset
Within 1 hour (for symptomatic relief).
Excretion
Mainly via feces (about 80%) and urine (about 10%).
Half life
Approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations in 2-3 hours.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to fexofenadine, terfenadine, or any component of the formulation.
Drug Interactions
Erythromycin or Ketoconazole
May increase fexofenadine plasma concentrations (though generally not clinically significant).
Aluminum- or Magnesium-containing Antacids
Decreased fexofenadine absorption if taken simultaneously. Separate administration by approximately 2 hours.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, and dry mouth. Management is symptomatic and supportive; no specific antidote is known. Hemodialysis does not effectively remove fexofenadine.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs the potential risk to the fetus. Fexofenadine is excreted in human breast milk; use with caution in nursing mothers and only if clearly needed.

Clinical Trials
Fexofenadine has undergone extensive clinical trials demonstrating its efficacy and safety for allergic rhinitis and chronic idiopathic urticaria. However, specific trials for a '1 mg syrup' formulation would be rare or specialized.
Lab Monitoring
- Generally, no routine laboratory monitoring is required.
Doctor Notes
- Emphasize the non-sedating nature, but advise patients about rare individual sensitivities.
- Educate patients about interactions with fruit juices and certain antacids.
- For this '1 mg' syrup, ensure clear instructions on administration volume if it differs significantly from standard Fexofenadine suspension dosing.
