Fenofibrate is a fibric acid derivative used to treat high cholesterol and triglyceride levels in the blood. It helps reduce the risk of cardiovascular diseases associated with dyslipidemia by improving the lipid profile.
Uses & Indications
Dosage
Adults
One tablet (10 mg) once daily, preferably with a main meal, or as directed by a healthcare professional. This specific strength might be for particular indications or as part of a specialized regimen.
Elderly
Dosage adjustments may be necessary based on renal function; consult physician. Generally, no specific dose adjustment required based on age alone.
Renal_impairment
Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73 m²). For moderate renal impairment (eGFR 30-59 mL/min/1.73 m²), a reduced dose may be necessary. Consult physician.
How to Take
To be taken orally, once daily with a main meal to enhance absorption and reduce gastrointestinal upset.
Mechanism of Action
Fenofibrate is a peroxisome proliferator-activated receptor alpha (PPARα) agonist. It activates PPARα, which in turn upregulates the synthesis of lipoprotein lipase and apolipoprotein A-I and A-II, while downregulating apolipoprotein C-III. This leads to increased catabolism of triglyceride-rich lipoproteins and reduction in VLDL production, thereby lowering triglyceride levels. It also moderately increases HDL cholesterol.
Pharmacokinetics
Onset
Peak plasma levels of the active metabolite, fenofibric acid, are achieved within 6-8 hours.
Excretion
Primarily excreted in the urine as fenofibric acid and its glucuronide conjugate.
Half life
Approximately 20-22 hours for fenofibric acid.
Absorption
Well absorbed, absorption is enhanced when taken with food.
Metabolism
Rapidly hydrolyzed to its active metabolite fenofibric acid by esterases. Not extensively metabolized by cytochrome P450 enzymes.
Side Effects
Contraindications
- •Severe renal impairment (eGFR < 30 mL/min/1.73 m²)
- •Severe hepatic impairment or active liver disease, including primary biliary cirrhosis
- •Pre-existing gallbladder disease
- •Hypersensitivity to fenofibrate or any component of the formulation
- •Known photoallergy or phototoxic reaction during treatment with fibrates or ketoprofen
- •Nursing mothers
Drug Interactions
Warfarin
May potentiate the anticoagulant effect of warfarin, requiring dose adjustment and INR monitoring.
Cyclosporine
Increased risk of renal dysfunction with concomitant use. Monitor renal function closely.
Bile Acid Sequestrants
May reduce fenofibrate absorption; administer fenofibrate at least 1 hour before or 4-6 hours after bile acid sequestrants.
Statins (HMG-CoA Reductase Inhibitors)
Increased risk of myopathy, rhabdomyolysis, and kidney failure, especially at higher doses or in patients with predisposing factors. Use with caution.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children. Do not freeze.
Overdose
In case of overdose, symptomatic and supportive measures should be taken. There is no specific antidote. Gastric lavage and general supportive care are recommended. Fenofibrate is not dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during lactation as fenofibrate and its metabolites are excreted into breast milk.
