Description
Overview of the medicine
Fexofenadine is a non-drowsy antihistamine used to relieve allergy symptoms such as sneezing, runny nose, itchy eyes, and skin rashes (urticaria).
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis and chronic idiopathic urticaria: 180 mg once daily.
Elderly
No specific dosage adjustment is generally required for elderly patients, but renal function should be monitored.
Renal_impairment
For adult patients with impaired renal function (creatinine clearance ≤ 80 mL/min), a starting dose of 60 mg once daily is recommended. For severe impairment, consult a physician.
How to Take
Take orally with water, preferably before a meal. Do not take with fruit juices.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It works by blocking the action of histamine, a natural substance in the body that causes allergic symptoms, without significantly penetrating the central nervous system, thus reducing sedative effects.
Pharmacokinetics
Onset
Onset of action is typically within 1 hour.
Excretion
Primarily excreted unchanged in the feces (approximately 80%) and urine (approximately 10-15%).
Half life
Plasma elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in approximately 2-3 hours.
Metabolism
Minimally metabolized; approximately 5% of the total dose is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to fexofenadine hydrochloride or any of the excipients.
- •Not recommended for children under 6 years of age for the 180 mg strength.
Drug Interactions
Erythromycin and Ketoconazole
Co-administration with erythromycin or ketoconazole has been shown to result in increased plasma levels of fexofenadine. Although clinical significance is generally minimal, caution is advised.
Antacids (containing aluminum and magnesium)
Concurrent administration of antacids containing aluminum and magnesium hydroxide within 15 minutes of fexofenadine can reduce fexofenadine absorption. Separate administration by at least 2 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, drowsiness, and dry mouth. Management is symptomatic and supportive; there is no specific antidote. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Use during pregnancy or lactation only if clearly needed and after consulting a physician, as adequate and well-controlled studies in pregnant or lactating women are lacking.
