Description
Overview of the medicine
Fexofenadine is a non-sedating antihistamine used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps alleviate sneezing, runny nose, itchy eyes/nose/throat, and skin rash/itching.
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis: 60 mg twice daily, or 180 mg once daily. For chronic idiopathic urticaria: 60 mg twice daily, or 180 mg once daily.
Elderly
No specific dose adjustment is recommended for elderly patients unless they have renal impairment.
Renal_impairment
For adults with impaired renal function (creatinine clearance ≤ 80 mL/min): initiate with 60 mg once daily.
How to Take
Take orally with water, with or without food. Avoid taking with fruit juices (e.g., grapefruit, orange, apple) as they may reduce the absorption and effectiveness of fexofenadine.
Mechanism of Action
Fexofenadine selectively antagonizes peripheral H1-receptors, thereby preventing the effects of histamine and reducing allergic symptoms.
Pharmacokinetics
Onset
Onset of action is approximately 1 hour.
Excretion
Approximately 80% is excreted in feces and 11% in urine, primarily as unchanged drug.
Half life
Elimination half-life ranges from 11 to 15 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in approximately 1-3 hours.
Metabolism
Minimally metabolized in the liver; approximately 5% of the dose undergoes hepatic metabolism.

Contraindications
- Hypersensitivity to fexofenadine or any of the excipients.
- Patients with known allergic reactions to similar antihistamines.
Drug Interactions
Fruit Juices
Fruit juices (grapefruit, orange, apple) can reduce the bioavailability of fexofenadine; avoid taking fexofenadine with these juices.
Erythromycin and Ketoconazole
Concomitant use with erythromycin or ketoconazole has been shown to increase fexofenadine plasma concentrations, but without significant increase in adverse effects.
Aluminum and Magnesium Hydroxide Antacids
Concomitant administration with aluminum- or magnesium-containing antacids may reduce the bioavailability of fexofenadine. Separate administration by at least 2 hours.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose may include dizziness, drowsiness, and dry mouth. Treatment should be symptomatic and supportive. Hemodialysis does not effectively remove fexofenadine from blood.

Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted into breast milk; caution should be exercised when administering to a nursing mother.
