Overview of the medicine
Filodox (Doxofylline) is a bronchodilator used for the treatment of bronchial asthma and chronic obstructive pulmonary disease (COPD). It helps to relax the muscles of the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
Adults: 400 mg twice daily. In some cases, 200 mg three times daily may be sufficient.
Elderly
Elderly patients may require a reduced dose. Initial dose 200 mg twice daily.
Renal_impairment
Use with caution. Dosage adjustment may be required in severe renal impairment.
How to Take
Filodox tablets should be taken orally, with or without food. Syrup can be taken with water.
Mechanism of Action
Doxofylline exerts its bronchodilator effect by selectively inhibiting phosphodiesterase enzymes, leading to an increase in intracellular cyclic AMP (cAMP) and subsequent relaxation of bronchial smooth muscles. Unlike theophylline, it has a lower affinity for adenosine receptors, resulting in fewer cardiac and central nervous system side effects.

Bronchodilation starts within 30-60 minutes.
Excretion
Mainly excreted in urine as metabolites, with a small portion as unchanged drug.

Approximately 6-8 hours in adults.
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached within 1 hour.
Metabolism
Extensively metabolized in the liver, primarily by oxidation.

- Hypersensitivity to doxofylline or other xanthine derivatives
- Acute myocardial infarction
- Severe hypotension
- Lactating women

Cimetidine
May increase doxofylline plasma levels.
Erythromycin
May increase doxofylline plasma levels, requiring dose reduction.
Phenobarbital, Rifampicin, Phenytoin
May decrease doxofylline plasma levels.

Symptoms of overdose include severe nausea, vomiting, epigastric pain, restlessness, insomnia, headache, tachycardia, arrhythmia, and convulsions. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal. Maintain airway and cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed. Contraindicated in lactating women.
