Overview of the medicine
Fix-A DS (Cefixime) is a third-generation cephalosporin antibiotic used to treat various bacterial infections. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Adults and children over 12 years or weighing more than 50 kg: 400 mg once daily or 200 mg every 12 hours for 7-14 days depending on the infection.
Elderly
No special dosage adjustment required unless severe renal impairment.
Renal_impairment
Dose adjustment required. CrCl 21-60 mL/min: 300 mg daily. CrCl < 20 mL/min: 200 mg daily.
How to Take
Fix-A DS tablets should be swallowed whole with water, with or without food. Taking it with food may reduce gastrointestinal discomfort.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Typically within 2-4 hours after oral administration.
Excretion
Mainly via renal excretion (glomerular filtration and tubular secretion) and some biliary excretion.
Half life
Average 3-4 hours, can be prolonged in renal impairment.
Absorption
Approximately 30-50% orally absorbed. Food can delay absorption but does not significantly affect the extent.

Not extensively metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or other cephalosporins
- History of severe hypersensitivity to penicillin (due to potential cross-reactivity)
Drug Interactions
Warfarin
May increase prothrombin time (PT) and INR, leading to increased bleeding risk. Monitor coagulation parameters closely.
Nifedipine
Concurrent use may increase Cefixime absorption.
Carbamazepine
May increase carbamazepine levels with concomitant use.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (though evidence is not strong for all antibiotics, advise caution).

Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
