Description
Overview of the medicine
Fixotim (Cefixime) is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
200-400 mg once or twice daily, depending on the severity of infection.
Elderly
No dose adjustment generally required unless there is severe renal impairment.
Renal_impairment
Dose adjustment required for creatinine clearance < 60 mL/min (e.g., 200 mg daily for CrCl 21-60 mL/min, 100 mg daily for CrCl < 20 mL/min).
How to Take
Can be taken with or without food. For oral suspension, shake well before use.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, ultimately leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved in 2-6 hours.
Excretion
Primarily excreted via the kidneys (urine) and bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slows absorption but does not significantly affect the extent.
Metabolism
Not significantly metabolized; about 50% of the absorbed dose is excreted unchanged in the urine.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or other cephalosporin antibiotics.
- •History of serious hypersensitivity reactions to penicillin.
Drug Interactions
Nifedipine
May increase cefixime concentrations.
Probenecid
May increase cefixime plasma levels by decreasing renal excretion.
Carbamazepine
May increase carbamazepine levels.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time; monitor INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Symptomatic and supportive treatment is recommended. Hemodialysis and peritoneal dialysis are not effective.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Small amounts excreted in breast milk; use with caution in nursing mothers.
