Overview of the medicine
Fluact 50 mg Capsule is primarily used to treat major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder, and bulimia nervosa. It belongs to a class of drugs known as Selective Serotonin Reuptake Inhibitors (SSRIs) that help restore the balance of serotonin in the brain.
Uses & Indications
Dosage
Adults
Major Depressive Disorder: Initially 20 mg once daily, may be increased to 60 mg daily. OCD: Initially 20 mg daily, may be increased to 60 mg daily. Bulimia Nervosa: 60 mg once daily. Panic Disorder: Initially 10 mg daily, increased to 20 mg daily after one week, maximum 60 mg daily. PMDD: 20 mg daily continuously or intermittently.
Elderly
Lower initial doses and slower titration may be considered (e.g., 10 mg daily initially), due to potential for decreased clearance and increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally, once daily in the morning, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluoxetine selectively inhibits the reuptake of serotonin by neurons in the central nervous system. This increases the concentration of serotonin in the synaptic cleft, thereby enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Clinical effects may take 2-4 weeks to become apparent.
Excretion
Excreted primarily through the kidneys (80%) and feces (15%), mainly as metabolites.
Half life
Fluoxetine: 1-3 days (acute); 4-6 days (chronic). Norfluoxetine (active metabolite): 4-16 days.
Absorption
Well absorbed from the gastrointestinal tract; peak plasma concentrations reached in 6-8 hours.
Metabolism
Extensively metabolized in the liver to an active metabolite, norfluoxetine, primarily by CYP2D6.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- Hypersensitivity to fluoxetine or any component of the formulation.
- Concomitant use with thioridazine or pimozide.

Increased risk of serotonin syndrome.
Warfarin, NSAIDs
Increased risk of bleeding due to effects on platelet aggregation.
TCAs, Pimozide, Thioridazine
Increased plasma levels and toxicity of these drugs.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms include nausea, vomiting, seizures, serotonin syndrome, cardiac dysfunction, and coma. Treatment is supportive and symptomatic; ensure airway, breathing, and circulation. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fluoxetine is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother. Consult your doctor.
