Overview of the medicine
Fluact 150 mg capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal and yeast infections. It is effective against candidiasis, cryptococcosis, and certain tinea infections.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg as a single oral dose. For other indications, dosage varies from 50 mg to 400 mg daily, duration depending on infection type.
Elderly
No dosage adjustment is generally required unless there is evidence of renal impairment.
Renal_impairment
For patients with creatinine clearance < 50 mL/min, the dose should be reduced by 50% after the initial loading dose.
How to Take
Take the capsule orally, with or without food. Swallow whole with water. Do not chew or crush.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is responsible for the demethylation of lanosterol to ergosterol. Ergosterol is an essential component of the fungal cell membrane; its impaired synthesis leads to increased cellular permeability and eventual cell death, exerting a fungistatic effect.

Onset
Clinical effects typically observed within 24-48 hours for acute infections, but full therapeutic effect may take several days depending on the infection type and severity.
Excretion
Primarily renal; approximately 80% of the dose is excreted unchanged in the urine, and about 11% as metabolites.
Half life
Approximately 30 hours, which allows for once-daily dosing.
Absorption
Well absorbed orally, with bioavailability greater than 90% (similar to intravenous administration). Peak plasma concentrations are achieved within 0.5 to 1.5 hours.
Metabolism
Minimally metabolized; about 11% of the drug is metabolized in the liver to inactive metabolites. It is a potent inhibitor of CYP2C9 and a moderate inhibitor of CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to fluconazole, other azole antifungals, or any excipients of the formulation.
- Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin due to risk of QT prolongation and serious arrhythmias.

Increased prothrombin time (PT) leading to bleeding risk.

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be initiated. Gastric lavage may be considered if appropriate. Hemodialysis can significantly reduce fluconazole plasma levels.
Pregnancy & Lactation
Pregnancy: Category D (for long-term/high-dose use) or C (single dose for vaginal candidiasis). Use during pregnancy should be avoided unless the benefits outweigh the potential risks. Lactation: Fluconazole is excreted in breast milk. Use is not recommended during breastfeeding, especially with high or repeated doses, unless strictly necessary.
