Description
Overview of the medicine
Fluact 50 mg Suspension is an antidepressant medication belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, and generalized anxiety disorder. It helps restore the balance of serotonin in the brain.
Uses & Indications
Dosage
Adults
Initial dose 50 mg once daily. May be increased to 100-150 mg daily, depending on clinical response and tolerability. Administer as a single dose in the morning or evening.
Elderly
A lower initial dose (e.g., 25 mg daily) and slower titration may be advisable. Maximum dose should be carefully considered.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. For severe impairment, caution is advised, and a lower dose or less frequent dosing may be considered.
How to Take
Administer orally, once daily. Shake the bottle well before use. Measure the prescribed dose using the provided measuring device (spoon or dropper). Can be taken with or without food.
Mechanism of Action
Fluact selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to an enhancement of serotonergic neurotransmission.
Pharmacokinetics
Onset
Initial antidepressant effects may be seen within 1-2 weeks, but full therapeutic effect may take 4-6 weeks.
Excretion
Excreted predominantly in the urine as metabolites, with a small amount excreted in feces.
Half life
Approximately 2-4 days for the parent compound, and 7-15 days for the active metabolite.
Absorption
Well absorbed orally; peak plasma concentrations achieved within 4-8 hours. Food may slightly delay absorption but does not significantly affect bioavailability.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6, to an active metabolite. Significant first-pass metabolism.
Side Effects
Contraindications
- Hypersensitivity to Fluact or any component of the suspension.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- Concomitant use with thioridazine or pimozide.
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes). Avoid concomitant use.
Tramadol
Increased risk of serotonin syndrome.
Warfarin
Increased risk of bleeding due to Fluact's potential to displace protein-bound warfarin.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
Tricyclic Antidepressants (TCAs)
May increase plasma concentrations of TCAs, requiring dosage adjustments.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, seizures, somnolence, tachycardia, and ECG changes. Management should be supportive and symptomatic. Gastric lavage or activated charcoal may be considered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to SSRIs late in the third trimester have developed complications requiring prolonged hospitalization, respiratory support, and tube feeding. Fluact is excreted in breast milk; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Periodic monitoring of electrolyte levels (especially sodium) if hyponatremia is suspected.
- Liver function tests in patients with pre-existing hepatic impairment or if symptoms suggest liver dysfunction.
Doctor Notes
- Advise patients about the delayed onset of therapeutic effects and the importance of continued treatment.
- Counsel patients on the risk of suicidal ideation, especially at the start of treatment or during dose adjustments.
- Assess for potential drug-drug interactions, particularly with other serotonergic agents, MAOIs, and drugs affecting coagulation.
