Overview of the medicine
Fluderm (Fludrocortisone Acetate) is a synthetic corticosteroid with potent mineralocorticoid properties and some glucocorticoid activity. It is primarily used as a replacement therapy for adrenocortical insufficiency, such as Addison's disease, and for salt-losing adrenogenital syndrome, to help regulate fluid and electrolyte balance.

Dosage
Adults
Typical dosage for Addison's disease: 0.1 mg daily, adjusted as needed. For POTS: 0.1 mg 1-3 times daily.
Elderly
Use with caution; dose adjustment may be necessary due to decreased renal/hepatic function and comorbidities.

No specific guidelines, but monitor electrolytes closely; dose adjustment may be required based on clinical response and electrolyte levels.
How to Take
Administer orally, usually once daily in the morning, with food to minimize gastrointestinal upset.
Mechanism of Action
Fludrocortisone acts as a potent mineralocorticoid, binding to mineralocorticoid receptors in the kidney tubules. This binding promotes the reabsorption of sodium and water from the renal tubules back into the blood, while simultaneously increasing the excretion of potassium and hydrogen ions. It also has some glucocorticoid effects, reducing inflammation and suppressing the immune system.

Contraindications

Storage

Symptoms of overdose include edema, hypertension, hypokalemia, and muscle weakness. Management is symptomatic and supportive, including correction of electrolyte imbalance. Discontinuation or dose reduction may be necessary.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Fludrocortisone is excreted in breast milk; caution should be exercised when administered to a nursing woman.
