Overview of the medicine
Flugal 150 mg Capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal infections, including vaginal candidiasis, oropharyngeal and esophageal candidiasis, and cryptococcal meningitis. It works by stopping the growth of fungi.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: Single oral dose of 150 mg. For other infections, dosage varies (e.g., 50-400 mg daily).
Elderly
No dose adjustment generally required unless renal function is impaired. Use caution.

Dose reduction necessary; e.g., for creatinine clearance <50 mL/min, reduce dose by 50% after initial loading dose.
How to Take
Take the capsule orally with or without food. For vaginal candidiasis, it's a single dose. For other conditions, follow your doctor's instructions.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is responsible for converting lanosterol to ergosterol. Ergosterol is an essential component of the fungal cell membrane; its inhibition leads to increased cell membrane permeability and eventual cell lysis.

Contraindications
- Hypersensitivity to Fluconazole or other azole antifungals
- Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin in patients receiving multiple doses of fluconazole

Warfarin
Increased prothrombin time, increased risk of bleeding.
Phenytoin
Increased phenytoin levels, risk of toxicity.
Hydrochlorothiazide
Increases plasma concentrations of fluconazole.
Cyclosporine, Tacrolimus
Increased levels of immunosuppressants, risk of nephrotoxicity.
Oral Hypoglycemics (e.g., sulfonylureas)
Increased blood glucose lowering effect, risk of hypoglycemia.

Symptoms of overdose may include hallucinations and paranoid behavior. Treatment is symptomatic and supportive. Gastric lavage may be performed if indicated. Hemodialysis significantly reduces plasma levels of fluconazole.

Pregnancy Category C (single dose for vaginal candidiasis might be Category B). Chronic use in pregnancy is contraindicated due to potential teratogenic effects. Use with caution during lactation; fluconazole is excreted in breast milk.
