Overview of the medicine
Fluking 250 mg Capsule contains Praziquantel, an anthelmintic agent used for the treatment of various trematode (fluke) and cestode (tapeworm) infections. It works by causing severe spasms and paralysis of the worms' musculature, leading to their detachment from host tissue and subsequent elimination from the body.
Uses & Indications
Dosage
Adults
The dosage varies depending on the type of infection. For Schistosomiasis: 40 mg/kg as a single dose or divided into two doses. For Clonorchiasis/Opisthorchiasis: 25 mg/kg three times daily for 1-3 days. For Taeniasis: 10-20 mg/kg as a single dose.
Elderly
No specific dose adjustment is generally required for elderly patients, but caution should be exercised due to potential age-related decrease in renal or hepatic function.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. For severe renal impairment, dose reduction may be considered due to potential accumulation of metabolites.
How to Take
Take the capsule whole with water, preferably with food to minimize gastrointestinal discomfort. Do not chew or crush the capsule.
Mechanism of Action
Praziquantel increases the permeability of the cell membranes of susceptible worms to calcium ions. This leads to massive contractions and paralysis of the musculature of the worms. The worms detach from the host's blood vessels and are then cleared from the body by immune responses or passage through the digestive system.

Within hours of administration, visible effects on worms may be observed.
Excretion
Mainly excreted in the urine (70-80%) and to a lesser extent in feces (10-20%) as metabolites.

Approximately 1-2.5 hours.

Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 enzymes (mainly CYP3A4), to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Praziquantel or any component of the formulation.
- Ocular cysticercosis (due to potential irreversible eye damage).
- Co-administration with strong CYP3A4 inducers (e.g., Rifampicin).
Drug Interactions
Rifampicin
Significantly decreases praziquantel plasma levels, reducing its therapeutic effect. Co-administration is contraindicated.
Phenytoin, Carbamazepine, Dexamethasone
May decrease praziquantel plasma concentrations.
Cimetidine, Ketoconazole, Grapefruit Juice
May increase praziquantel plasma concentrations.
