Overview of the medicine
Flustaph 250 mg Capsule contains Flucloxacillin, an antibiotic belonging to the penicillin group. It is used to treat various bacterial infections, particularly those caused by penicillinase-producing staphylococci, such as skin and soft tissue infections, bone and joint infections, and respiratory tract infections.
Uses & Indications
Dosage
Adults
250-500 mg every 6 hours, usually taken 30-60 minutes before meals. Doses up to 1 g every 6 hours may be used for severe infections.
Elderly
Similar to adult dosage, but dose reduction may be considered in severe renal impairment.
Renal_impairment
Dosage adjustment required. For severe impairment (creatinine clearance <10 mL/min), consider 250 mg every 8-12 hours.
How to Take
Take orally, preferably 30-60 minutes before meals, with a glass of water, to ensure optimal absorption. Do not open or chew the capsule.
Mechanism of Action
Flucloxacillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is resistant to degradation by penicillinase, an enzyme produced by some bacteria that would otherwise inactivate other penicillins.

Onset
Typically within 1-2 hours.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 0.5-1.5 hours in adults with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract, though absorption is reduced by food. Peak plasma concentrations are reached within 1-2 hours after oral administration.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications

- •Infectious mononucleosis (increased risk of rash)
Drug Interactions
Warfarin
May enhance anticoagulant effect. Monitor INR closely.
Probenecid
Increases and prolongs flucloxacillin levels.
Methotrexate
May increase methotrexate plasma concentrations, leading to increased toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives; advise additional contraception.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, diarrhea, and electrolyte imbalance. Treatment is largely symptomatic and supportive. Gastric lavage or activated charcoal may be considered if recent. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy: Category B. Generally considered safe, but use with caution. Lactation: Excreted in breast milk in small amounts; generally considered safe, but monitor infant for potential side effects like diarrhea or rash.
