Description
Overview of the medicine
Fondy (Fondazepam) is a novel antidepressant and anxiolytic agent primarily used for the treatment of major depressive disorder and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose: 25 mg once daily. May be increased to 50 mg daily after 1-2 weeks if needed. Maximum recommended dose is 100 mg daily.
Elderly
Start with 12.5 mg once daily. Max 50 mg daily. Adjust dosage cautiously due to potential for reduced renal/hepatic function.
Renal_impairment
No dose adjustment needed for mild to moderate impairment. Use with caution in severe renal impairment, consider lower doses.
How to Take
Fondy tablets should be taken orally once daily, with or without food, preferably at the same time each day.
Mechanism of Action
Fondazepam selectively inhibits the reuptake of serotonin in the brain, thereby increasing the concentration of serotonin in the synaptic clefts, which contributes to its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial effects may be noticed within 1-2 weeks, full antidepressant effect within 4-6 weeks.
Excretion
Excreted primarily via urine as metabolites, with a small portion excreted in feces.
Half life
Approximately 20-24 hours.
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations achieved within 2-4 hours.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (CYP2D6, CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to Fondazepam or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs).
- •Uncontrolled narrow-angle glaucoma.
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, severe agitation, delirium, coma). Should not be used within 14 days of starting or discontinuing an MAOI.
NSAIDs
Increased risk of gastrointestinal bleeding.
Warfarin
May potentiate the anticoagulant effect of warfarin, increasing bleeding risk. Monitor INR.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, nausea, vomiting, tachycardia, tremor, and agitation. Management is symptomatic and supportive, including maintaining a clear airway, monitoring cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during breastfeeding as Fondazepam may be excreted in breast milk.
