Description
Overview of the medicine
Cefepime is a broad-spectrum, fourth-generation cephalosporin antibiotic used to treat various bacterial infections, including those resistant to other antibiotics. It is administered by injection.
Uses & Indications
Dosage
Adults
1-2 gm IV/IM every 8-12 hours, depending on the severity and type of infection. Max 2 gm every 8 hours.
Elderly
Dosage adjustment is usually required based on renal function due to age-related decline in kidney function.
Renal_impairment
Significant dose reduction required based on creatinine clearance (e.g., for CrCl 30-60 mL/min, 1 gm every 24 hours).
How to Take
Fulspec 1 gm injection is typically reconstituted with sterile water for injection or other compatible diluents. It is then administered by deep intramuscular (IM) injection or by intravenous (IV) infusion over 30 minutes.
Mechanism of Action
Cefepime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It has a high affinity for PBPs and stability against beta-lactamases.
Pharmacokinetics
Onset
Rapid onset of action, typically within minutes after IV administration.
Excretion
Primarily renal excretion via glomerular filtration.
Half life
Approximately 2 hours in individuals with normal renal function.
Absorption
Rapid and complete absorption after intravenous (IV) or intramuscular (IM) administration.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to cefepime or any other cephalosporin antibiotics.
- •Immediate-type hypersensitivity reaction (e.g., anaphylaxis, severe skin reaction) to any beta-lactam antibiotic (e.g., penicillins, carbapenems).
Drug Interactions
Loop Diuretics
May increase cefepime concentrations and the risk of adverse effects.
Aminoglycosides
Increased risk of nephrotoxicity and ototoxicity when co-administered with cefepime.
Oral Anticoagulants (e.g., Warfarin)
Increased INR/prothrombin time in some patients, requiring monitoring.
Vaccines (e.g., Live bacterial vaccines)
Antibiotics may decrease the therapeutic effect of live bacterial vaccines.
Storage
Store intact vials at controlled room temperature (20-25°C) and protect from light. Reconstituted solutions should be used immediately or stored as per manufacturer's guidelines.
Overdose
In case of overdose, symptoms may include encephalopathy (e.g., confusion, hallucinations, stupor, coma), myoclonus, and seizures. Treatment is symptomatic and supportive. Hemodialysis may be useful in removing cefepime from the system.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use during pregnancy only if clearly needed. Cefepime is excreted in small amounts in human milk; use with caution during lactation.
