Description
Overview of the medicine
Fulspec-250 mg injection contains Fulvestrant, which is an estrogen receptor antagonist indicated for the treatment of hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced or metastatic breast cancer.
Uses & Indications
Dosage
Adults
500 mg administered intramuscularly into the buttock slowly (1-2 minutes per injection) on Day 1, Day 15, Day 29, and once monthly thereafter. The 500 mg dose requires two separate 250 mg injections.
Elderly
No dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is recommended for patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
For intramuscular injection only. Administer slowly (1-2 minutes) into the buttock. Alternate buttocks for subsequent injections.
Mechanism of Action
Fulvestrant is a selective estrogen receptor degrader (SERD) that binds to the estrogen receptor (ER) and downregulates its expression. It blocks and degrades the estrogen receptor, inhibiting estrogen's proliferative effects on breast cancer cells.
Pharmacokinetics
Onset
Steady state plasma concentrations are achieved after 3-6 monthly doses.
Excretion
Primarily excreted in feces (approximately 90%) after hepatic metabolism, with minimal renal excretion.
Half life
Approximately 40 days.
Absorption
Slow and prolonged absorption following intramuscular injection. Bioavailability is about 23-40%.
Metabolism
Extensively metabolized, primarily via CYP3A4, but also by other non-CYP pathways.
Side Effects
Contraindications

Drug Interactions
Warfarin
Potential for increased INR; monitor coagulation parameters closely.
CYP3A4 Inducers/Inhibitors
Fulvestrant is primarily metabolized by CYP3A4, but significant drug interactions are not expected due to its long half-life and low clearance.
Storage
Store in a refrigerator (2°C to 8°C). Do not freeze. Keep in the original carton to protect from light.
Overdose
There is no specific antidote for Fulvestrant overdose. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Contraindicated in pregnancy due to potential fetal harm. Avoid during lactation as it is unknown if Fulvestrant is excreted in human milk.
