Uses & Indications
Dosage
Adults
500 mg to 1 g intravenously or intramuscularly every 8-12 hours, depending on the severity of infection.
Elderly
No specific dose adjustment typically required based on age alone, but renal function should be monitored.
Renal_impairment
For creatinine clearance <30 mL/min, dosage adjustment (e.g., 250 mg every 12 hours) may be necessary. Close monitoring of drug levels is advised.
How to Take
Fulspec 500 mg Injection can be administered via slow intravenous (IV) infusion over 30-60 minutes or deep intramuscular (IM) injection. For IM, reconstitute with sterile water for injection or 0.9% sodium chloride solution. For IV, dilute further after reconstitution.
Mechanism of Action
The active ingredient in Fulspec 500 mg Injection inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death. It possesses a high affinity for key PBPs in susceptible organisms.

Within minutes for intravenous administration; within 30-60 minutes for intramuscular administration.
Excretion
Mainly excreted via the kidneys (glomerular filtration and tubular secretion) and a minor portion through bile. Approximately 70-80% excreted unchanged in urine within 24 hours.

Approximately 2-3 hours in healthy adults, prolonged in renal impairment.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved shortly after IV infusion.

Side Effects
Contraindications

Drug Interactions
Probenecid
May increase and prolong the plasma concentration of Fulspec by inhibiting renal tubular secretion.
Aminoglycosides

Oral Anticoagulants (e.g., Warfarin)
May potentiate the anticoagulant effect. Monitor INR/PT more frequently.
Storage
Store below 30°C in a dry place, away from light and moisture. Do not freeze. Reconstituted solution should be used immediately or within 6-12 hours if refrigerated.
Overdose
Overdose may lead to neurological symptoms including convulsions, especially in patients with renal impairment. Treatment is symptomatic and supportive. Hemodialysis may be useful in reducing plasma concentrations.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of fetal harm, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Excreted in low concentrations into breast milk; caution is advised when administering to a nursing mother.
