Overview of the medicine
Fluconazole is an antifungal medication used to treat a variety of fungal infections. It belongs to the triazole class of antifungals.
Uses & Indications
Dosage
Adults
For oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. For prevention of fungal infections in immunocompromised patients: 50-400 mg once daily.
Elderly
Dosage adjustments usually not necessary unless severe renal impairment.
Renal_impairment
Dosage reduction may be required based on creatinine clearance. For CrCl <50 mL/min, 50% of the usual dose.
How to Take
Can be taken with or without food. Take the capsule whole with water.
Mechanism of Action
Inhibits fungal cytochrome P450-dependent enzyme lanosterol 14-alpha-demethylase, which is essential for ergosterol synthesis. Ergosterol is a vital component of fungal cell membranes.

Varies by infection type, generally symptomatic relief within days.
Excretion
Primarily renal excretion as unchanged drug (about 80% dose).
Half life
Approximately 30 hours.
Absorption
Well absorbed orally, bioavailability >90%. Peak plasma concentrations within 0.5-1.5 hours.
Metabolism
Minimally metabolized by the liver (about 11% dose).
Side Effects
Contraindications
- •Hypersensitivity to fluconazole or other azole antifungals.
- •Concomitant use with terfenadine (if multiple doses of fluconazole 400 mg/day or more).
- •Concomitant use with cisapride, astemizole, pimozide, quinidine, erythromycin.
Drug Interactions
Statins
Increased risk of myopathy/rhabdomyolysis.
Warfarin
Increased prothrombin time; monitor INR.
Phenytoin
Increased phenytoin levels; monitor levels.
Rifampicin
Decreased fluconazole levels.
Cyclosporine, Tacrolimus
Increased levels of immunosuppressants; monitor levels.
Benzodiazepines (e.g., Midazolam)
Increased benzodiazepine levels.
Oral Hypoglycemics (sulfonylureas)
Increased plasma concentrations; monitor blood glucose.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptomatic and supportive measures. Hemodialysis significantly reduces plasma levels.
Pregnancy & Lactation
Pregnancy Category C (single dose) or D (chronic/high dose); excreted in breast milk. Consult doctor before use.
