Overview of the medicine
Funzole (Fluconazole) is an antifungal medication used to treat various fungal infections, including vaginal candidiasis, oral thrush, esophageal candidiasis, and systemic fungal infections.
Uses & Indications
Dosage
Adults
Vaginal candidiasis: 150 mg single oral dose. Oropharyngeal candidiasis: 50-200 mg once daily for 7-14 days. Systemic candidiasis: 400 mg on day 1, then 200-400 mg once daily. Cryptococcal meningitis: 400 mg on day 1, then 200-400 mg once daily for 10-12 weeks after CSF culture becomes negative.
Elderly
No dosage adjustment usually required unless there is renal impairment.
Renal_impairment
Dosage adjustment needed for creatinine clearance < 50 mL/min (e.g., 50% of recommended dose for 20-50 mL/min, 25% for < 20 mL/min).
How to Take
Oral capsules/suspension can be taken with or without food. IV infusion should be administered over 1-2 hours.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is essential for the conversion of lanosterol to ergosterol. Ergosterol is a vital component of the fungal cell membrane, and its depletion leads to increased cell membrane permeability and cell death.
Pharmacokinetics
Onset
Within 24 hours for vaginal candidiasis, longer for other systemic infections.
Excretion
Mainly renal, with about 80% of the dose excreted unchanged in urine.
Half life
Approximately 30 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally (over 90%), peak plasma concentrations reached within 1-2 hours. Bioavailability is not affected by food.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to fluconazole or other azole antifungals
- Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin (due to risk of QT prolongation and torsades de pointes)
Drug Interactions
Warfarin
Increased prothrombin time, monitor INR.
Phenytoin
Increased plasma concentrations, monitor phenytoin levels.
Rifampicin
Decreased fluconazole levels, consider increasing fluconazole dose.
Sulfonylureas
Increased plasma concentrations, monitor glucose.
Hydrochlorothiazide
Increased fluconazole levels.
Cyclosporine, Tacrolimus
Increased plasma concentrations, monitor levels.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Treatment is symptomatic and supportive; gastric lavage may be considered. Hemodialysis significantly reduces plasma concentrations.
Pregnancy & Lactation
Pregnancy Category D (single dose for vaginal candidiasis is C). Should be avoided during pregnancy unless benefits outweigh risks. Small amounts excreted in breast milk; use with caution.
