Description
Overview of the medicine
Furex 500 mg Tablet contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
For most infections, 250 mg to 500 mg twice daily. For uncomplicated urinary tract infections, 125 mg to 250 mg twice daily. For uncomplicated gonorrhea, a single dose of 1000 mg.
Elderly
No special precautions required, dose as per adult dosage, but monitor renal function.
Renal_impairment
Dosage adjustment is necessary. For creatinine clearance 10-29 mL/min, 250 mg once daily. For creatinine clearance <10 mL/min, 125 mg once daily. Hemodialysis patients should take an additional dose at the end of dialysis.
How to Take
Take orally, preferably with food, for optimal absorption. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations reached in 2-3 hours.
Excretion
Primarily excreted unchanged by glomerular filtration and tubular secretion via the kidneys.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and hydrolyzed to cefuroxime. Enhanced absorption when taken with food.
Metabolism
Not significantly metabolized in the liver; hydrolyzed in the intestinal mucosa and blood to active cefuroxime.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins).
Drug Interactions
Warfarin
Cephalosporins may enhance the anticoagulant effect of warfarin.
Probenecid
Increases cefuroxime plasma concentrations and prolongs its half-life.
Oral contraceptives
May reduce the efficacy of oral contraceptives; additional birth control methods recommended.
Antacids/H2-receptor antagonists
May decrease the bioavailability of cefuroxime axetil, especially if taken within a few hours.
Loop diuretics (e.g., Furosemide)
Concomitant use with high doses of cephalosporins may increase the risk of nephrotoxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include convulsions, encephalopathy, and neuro-muscular excitability. Treatment is symptomatic and supportive; hemodialysis or peritoneal dialysis may be considered.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in low concentrations in breast milk, generally considered safe but monitor infant for potential adverse effects (e.g., diarrhea, candidiasis).

- Liver function tests (e.g., AST, ALT) occasionally.
- Complete blood count (CBC) during prolonged therapy.
Doctor Notes
- Advise patients to report any signs of allergic reaction or severe diarrhea immediately.
- Consider renal function adjustment in elderly or renally impaired patients.
- Emphasize the importance of completing the full antibiotic course.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if you feel better, to prevent recurrence and development of antibiotic resistance.
- Take the tablet with food to enhance absorption and reduce stomach upset.
