Description
Overview of the medicine
Furosemide is a potent loop diuretic used to treat fluid retention (edema) associated with congestive heart failure, liver disease, kidney disease, and high blood pressure. This 750 mg injection is typically reserved for severe cases or continuous intravenous infusion where high doses are required for diuresis.
Uses & Indications
Dosage
Adults
Initial dose 20-40 mg IV, repeated every 6-8 hours. For severe edema, continuous infusion of 750 mg over 24 hours might be used, adjusted based on patient response and urine output.
Elderly
Lower initial doses and slower titration may be required due to increased sensitivity and potential for dehydration and electrolyte imbalance. Close monitoring is essential.
Renal_impairment
Dosage adjustment is often necessary. Higher doses may be required in severe renal failure to achieve adequate diuresis, but careful monitoring of electrolytes and renal function is crucial to avoid ototoxicity and worsening renal function. Doses up to 1500-2000 mg/day have been used in continuous infusion.
How to Take
For intravenous use only. Administer slowly over 1-2 minutes for bolus doses or via continuous infusion at a controlled rate.
Mechanism of Action
Furosemide acts on the thick ascending limb of the loop of Henle in the kidney, inhibiting the Na-K-2Cl cotransporter. This prevents the reabsorption of sodium, potassium, and chloride ions, leading to increased excretion of water, sodium, chloride, and other electrolytes.
Pharmacokinetics
Onset
Within 5 minutes after intravenous administration.
Excretion
Primarily renal excretion (80-90% unchanged in urine).
Half life
Approximately 1-1.5 hours in healthy adults, prolonged in renal impairment.
Absorption
Rapidly absorbed after intravenous administration, 100% bioavailability.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Anuria (absence of urine production).
- Hypersensitivity to furosemide or sulfonamides.
- Hepatic coma or precoma.
- Severe electrolyte depletion.
Drug Interactions
NSAIDs
May reduce diuretic and antihypertensive effects, increased risk of renal toxicity.
Digoxin
Increased risk of digitalis toxicity due to hypokalemia.
Lithium
Increased lithium levels due to reduced renal clearance.
Corticosteroids
Increased risk of hypokalemia.
Aminoglycoside antibiotics
Increased risk of ototoxicity.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms include dehydration, electrolyte imbalance (hypokalemia, hyponatremia, hypochloremia, metabolic alkalosis), hypovolemia, hypotension, and cardiovascular collapse. Management involves fluid and electrolyte replacement, and symptomatic and supportive care.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Furosemide is excreted in breast milk and may inhibit lactation; caution is advised during breastfeeding.

Lab Monitoring
- Serum electrolytes (sodium, potassium, chloride, magnesium, calcium) regularly.
- Renal function (creatinine, BUN).
- Blood pressure and heart rate.
Doctor Notes
- Monitor fluid status, electrolytes, and renal function closely, particularly in patients receiving high doses or with pre-existing renal impairment. Consider continuous infusion for optimal diuresis in severe cases.
- Beware of potential drug interactions, especially with aminoglycosides and lithium. Educate patients on symptoms of dehydration and electrolyte imbalance.
Patient Guidelines
- Report any signs of dehydration (e.g., extreme thirst, dry mouth, decreased urination) or electrolyte imbalance (e.g., muscle weakness, cramps, irregular heartbeat).
- Avoid sudden changes in posture to prevent dizziness.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness or lightheadedness, especially at the start of treatment or with dose changes. Avoid driving or operating machinery until you know how Furosemide affects you.
Lifestyle Advice
- Maintain adequate fluid intake unless advised otherwise by your doctor.
- Avoid excessive salt intake.
- Monitor your weight regularly.
