G-Fix 400 mg Capsule is an antibiotic medicine containing Cefixime, used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, and ears.
Uses & Indications
Dosage
Adults
Adults: 400 mg once daily or 200 mg every 12 hours, typically for 7-14 days depending on infection severity and type.
Elderly
No specific dose adjustment required unless there is significant renal impairment.
Renal_impairment
CrCl 21-60 mL/min: 300 mg daily. CrCl < 20 mL/min: 200 mg daily.
How to Take
Oral. Can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Cefixime is a cephalosporin antibiotic that works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and also through bile/feces.
Half life
Average 3-4 hours, may be prolonged in renal impairment.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the total amount absorbed.
Metabolism
Not extensively metabolized; approximately 10% metabolised in the liver.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or other cephalosporins.
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to any type of beta-lactam antibacterial agents (e.g., penicillins).
Drug Interactions
Nifedipine
May increase Cefixime peak plasma levels and AUC.
Carbamazepine
Elevated Carbamazepine levels reported with concomitant use.
Oral anticoagulants (e.g., Warfarin)
May increase prothrombin time/INR. Monitor coagulation parameters.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. Hemodialysis or peritoneal dialysis is not substantially effective in removing Cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
