Description
Overview of the medicine
Salbutamol is a selective short-acting beta-2 adrenergic receptor agonist used for the relief of bronchospasm in conditions such as asthma and chronic obstructive pulmonary disease (COPD).
Uses & Indications
Dosage
Adults
4 mg (10 ml) 3 or 4 times daily. May be increased to 8 mg (20 ml) if necessary.
Elderly
Initially 2 mg (5 ml) 3 or 4 times daily, increasing if necessary.
Renal_impairment
Dosage adjustment may be necessary in severe renal impairment, consult physician.
How to Take
To be taken orally. Can be taken with or without food.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors primarily located in the bronchial smooth muscle. This stimulation leads to the activation of adenyl cyclase, which increases intracellular cyclic AMP (cAMP) levels. Increased cAMP results in the relaxation of bronchial smooth muscle, leading to bronchodilation.
Pharmacokinetics
Onset
5-15 minutes (oral)
Excretion
Excreted primarily in the urine as metabolites and unchanged drug.
Half life
4-6 hours
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations within 2-3 hours.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to Salbutamol or any component of the syrup
- Threatened abortion during the first or second trimesters of pregnancy
Drug Interactions
Digoxin
May increase serum digoxin levels.
Beta-blockers (e.g., Propranolol)
Can antagonize the effects of Salbutamol and cause severe bronchospasm.
Diuretics (non-potassium-sparing)
May exacerbate hypokalemia when co-administered with high doses of Salbutamol.
Monoamine Oxidase Inhibitors (MAOIs) & Tricyclic Antidepressants (TCAs)
May potentiate the vascular effects of Salbutamol.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include tachycardia, tremor, headache, hypokalemia, and hyperglycemia. Treatment is symptomatic and supportive; consider cardioselective beta-blockers with caution for cardiac symptoms.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk, use with caution during lactation.
