Overview of the medicine
Gaspro 40 mg Injection contains Pantoprazole, a proton pump inhibitor, used to reduce stomach acid production for conditions like ulcers and reflux. It is administered intravenously.
Uses & Indications
Dosage
Adults
For GERD: 40 mg once daily. For Zollinger-Ellison Syndrome: Initial 80 mg once daily, adjusted based on acid output. For ulcer rebleeding prevention: 80 mg bolus, then 8 mg/hr continuous infusion for 72 hours.
Elderly
No specific dose adjustment required for elderly patients with normal renal and hepatic function.
Renal_impairment
No dose adjustment necessary for patients with renal impairment.
How to Take
Intravenous administration only. Administer as a slow injection over at least 2 minutes or as a short intravenous infusion over 15 minutes. It should be reconstituted with the provided solvent and further diluted if needed.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, inhibiting the final step of acid secretion.

Excretion
Mainly renal (approximately 80%) as metabolites, with the remainder excreted in the feces.
Half life
Approximately 1 hour (terminal elimination half-life for acid suppression effect is longer, 7-9 hours).

Rapidly absorbed after intravenous administration, 100% bioavailability.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications

Drug Interactions
Digoxin
Increased absorption of digoxin due to increased gastric pH.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Methotrexate
Increased plasma levels of methotrexate, especially at high doses, leading to potential toxicity.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption of drugs requiring acidic gastric pH.
Atazanavir, Nelfinavir (and other HIV protease inhibitors)
Reduced absorption due to altered gastric pH, potentially leading to loss of virological response.
Storage
Store below 25°C in a dry place, protected from light. Do not freeze. Reconstituted solution should be used within 6 hours at room temperature or 24 hours if refrigerated.
Overdose
Symptoms of overdose are generally unknown. In case of suspected overdose, symptomatic and supportive treatment should be given. Pantoprazole is not removed by hemodialysis.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and potential benefits outweigh risks. Caution advised during lactation as pantoprazole is excreted in breast milk.
