Description
Overview of the medicine
Gbpoietin 2000 IU Injection contains Epoetin alfa, a recombinant human erythropoietin. It is used to treat anemia associated with chronic kidney disease, chemotherapy, or certain other conditions by stimulating red blood cell production.
Uses & Indications
Dosage
Adults
Dosage varies by indication. For CKD, typically 50-100 IU/kg three times weekly initially, adjusted based on hemoglobin response. For chemotherapy-induced anemia, 150-300 IU/kg three times weekly or 40,000-60,000 IU once weekly.
Elderly
No specific dosage adjustment recommended for elderly patients, but monitoring for adverse effects is crucial.
Renal_impairment
Primary indication for use. Dosage adjusted based on hemoglobin levels and individual response.
How to Take
Administer by intravenous (IV) or subcutaneous (SC) injection. Do not shake the vial/syringe.
Mechanism of Action
Epoetin alfa stimulates erythropoiesis by binding to the erythropoietin receptor on erythroid progenitor cells, promoting their proliferation and differentiation into mature red blood cells.
Pharmacokinetics
Onset
Erythropoietic response typically begins within 1-2 weeks, with significant increases in hemoglobin seen after 2-6 weeks.
Excretion
Mainly cleared by non-renal routes, with a small percentage excreted in the urine.
Half life
Intravenous: Approximately 4-13 hours. Subcutaneous: Approximately 13-24 hours.
Absorption
Following subcutaneous (SC) administration, absorption is slow and prolonged. Peak serum concentrations are reached after 5-24 hours. Following intravenous (IV) administration, peak levels are immediate.
Metabolism
Primarily metabolized in the liver and kidney via proteolytic degradation.
Side Effects
Contraindications
- Uncontrolled hypertension.
- Pure red cell aplasia (PRCA) unrelated to Epoetin alfa therapy.
- Known hypersensitivity to Epoetin alfa, albumin (human), or any excipient.
- Patients with severe coronary artery disease or peripheral vascular disease with a history of thrombotic events.
Drug Interactions
No significant direct drug-drug interactions reported
Epoetin alfa is generally not known to have significant direct drug-drug interactions. However, iron supplements are often co-administered and their need should be monitored.
Storage
Store in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light. Do not shake.
Overdose
Overdose can result in polycythemia. If polycythemia occurs, phlebotomy may be performed. Discontinue Epoetin alfa temporarily until hemoglobin levels return to target range. Provide supportive care.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown if Epoetin alfa is excreted in human milk; caution is advised when administering to a nursing mother.

- Hemoglobin and hematocrit levels (at least weekly initially, then regularly)
- Blood pressure monitoring
- Iron status (serum ferritin, transferrin saturation)
Doctor Notes
- Careful monitoring of hemoglobin levels (target 10-11 g/dL), blood pressure, and iron status is essential.
- Initiate iron supplementation if iron stores are insufficient.
- Adjust dosage to maintain hemoglobin within the target range, avoiding rapid increases or exceeding target levels to mitigate cardiovascular risks.
Patient Guidelines
- Do not shake the vial or syringe vigorously, as this can denature the glycoprotein and reduce its activity.
- Store refrigerated at 2-8°C (36-46°F); do not freeze. Protect from light.
- Do not reuse or share needles/syringes. Dispose of used syringes properly.
