Description
Overview of the medicine
Geflox 500 mg Tablet is an antibiotic medicine containing Gatifloxacin, used to treat a variety of bacterial infections in adults. It belongs to the fluoroquinolone class of antibiotics.
Uses & Indications
Dosage
Adults
Typically 400 mg once daily for 7-10 days, or as directed by physician. For 500mg, it implies a higher dose or specific condition, often 500mg once daily for severe cases or specific infections.
Elderly
No specific dose adjustment is required for elderly patients with normal renal function. Monitor renal function.
Renal_impairment
Dose adjustment required for creatinine clearance < 40 mL/min. For CrCl 30-49 mL/min: 200 mg once daily. For CrCl < 30 mL/min: Initial 400 mg then 200 mg every 24 hours.
How to Take
Oral. Can be taken with or without food. Swallow tablet whole with water. Do not chew or crush.
Mechanism of Action
Gatifloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to the disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Onset of antibacterial action within hours.
Excretion
Predominantly excreted unchanged in the urine, with a small amount excreted in feces.
Half life
Elimination half-life is approximately 7 to 14 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration, with peak plasma concentrations occurring within 1-2 hours.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Gatifloxacin or other fluoroquinolones
- History of tendonitis or tendon rupture associated with quinolone use
- Patients with uncorrected hypokalemia
- Patients with known prolongation of the QT interval or uncorrected hypokalemia
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsions.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Insulin or oral hypoglycemic agents
May cause hypoglycemia or hyperglycemia. Monitor blood glucose closely.
Antacids containing aluminum or magnesium, sucralfate, iron supplements, multivitamin with zinc
Reduce oral absorption of Gatifloxacin. Administer Gatifloxacin 2 hours before or 4 hours after these agents.
Antiarrhythmics (Class IA and Class III), tricyclic antidepressants, macrolides, antipsychotics
Potential for QT interval prolongation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be considered. Supportative care, including hydration, and monitoring of ECG (due to QT prolongation risk) and vital signs should be provided. Hemodialysis is not effective for removing Gatifloxacin.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during breastfeeding as Gatifloxacin is excreted in breast milk and may harm the infant.

Lab Monitoring
- Blood glucose levels (especially in diabetic patients)
- Renal function (creatinine clearance)
- ECG (in patients at risk for QT prolongation)
- Liver function tests (if signs of hepatotoxicity develop)
Doctor Notes
- Counsel patients about the risk of tendinitis and tendon rupture, and advise them to discontinue treatment and seek medical attention if symptoms occur.
- Monitor blood glucose carefully in diabetic patients.
- Avoid use in patients with a history of QT prolongation or uncorrected hypokalemia.
- Ensure adequate hydration to prevent crystalluria.
Patient Guidelines
- Complete the full course of antibiotic, even if symptoms improve.
- Stay well-hydrated.
- Avoid excessive sun exposure.
- Report any pain or swelling in tendons immediately.
- Avoid driving or operating machinery if dizziness or visual disturbances occur.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness, lightheadedness, or visual disturbances. Patients should be cautioned against operating hazardous machinery or driving until they are reasonably certain that Gatifloxacin does not affect them adversely.
Lifestyle Advice
- Maintain good hygiene to prevent reinfection.
- Discuss all medications, supplements, and medical conditions with your doctor before starting treatment.
