Description
Overview of the medicine
Hitflam (generic name: hitflam) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and inflammation associated with various conditions such as arthritis, acute gout, and dysmenorrhea.
Uses & Indications
Dosage
Adults
Usually 50 mg to 100 mg daily in divided doses. Max 150 mg/day for short-term use.
Elderly
Lower doses may be required, monitor for adverse effects due to increased sensitivity and potential renal impairment.
Renal_impairment
Use with caution. Dose reduction or extended dosing intervals may be necessary in moderate to severe renal impairment. Avoid in severe renal failure.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Swallow whole, do not crush or chew prolonged-release forms.
Mechanism of Action
Hitflam acts by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By reducing prostaglandin synthesis, hitflam provides analgesic, anti-inflammatory, and antipyretic effects.
Pharmacokinetics
Onset
Pain relief usually starts within 30 minutes to 1 hour.
Excretion
Mainly excreted via urine (approximately 60%) and bile (approximately 35%) as metabolites.
Half life
Approximately 1-2 hours (terminal half-life).
Absorption
Rapidly and completely absorbed after oral administration, peak plasma concentrations typically achieved within 2-3 hours.
Metabolism
Extensively metabolized in the liver, primarily via hydroxylation and subsequent glucuronidation. Metabolites are mostly inactive.
Side Effects
Contraindications
- Hypersensitivity to hitflam or other NSAIDs
- Active peptic ulcer or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery in perioperative setting
Drug Interactions
Lithium
May increase lithium plasma levels, leading to toxicity.
Methotrexate
May increase methotrexate levels and toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Diuretics and ACE inhibitors/ARBs
May reduce the natriuretic and antihypertensive effects, and increase the risk of renal impairment.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Rare cases of acute renal failure, hypertension, respiratory depression, and coma have occurred. Management is supportive and symptomatic, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Avoid use during the third trimester of pregnancy due to the risk of premature closure of the fetal ductus arteriosus. Use with caution during early pregnancy and lactation only if the potential benefits outweigh the risks. Consult a doctor.

YesClinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of hitflam in various pain and inflammatory conditions. Post-marketing surveillance continues to monitor for rare adverse events.
Lab Monitoring
- Complete blood count (CBC)
- Liver function tests (LFTs)
- Renal function tests (RFTs)
- Blood pressure monitoring (especially during long-term therapy)
Doctor Notes
- Advise patients to report any unusual bleeding, dark stools, or persistent stomach pain.
- Monitor renal and hepatic function, and blood pressure regularly, especially in elderly patients or those on long-term therapy.
- Consider co-prescription of a proton pump inhibitor for patients at high gastrointestinal risk.
- Educate patients on the signs and symptoms of cardiovascular and gastrointestinal adverse events.
Patient Guidelines
- Take hitflam exactly as prescribed by your doctor.
- Always take with food or milk to minimize stomach upset.
- Do not exceed the recommended dose or duration of treatment.
- Report any signs of stomach bleeding (e.g., black, tarry stools, vomiting blood) to your doctor immediately.
- Avoid concomitant use with other NSAIDs or aspirin without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
