Overview of the medicine
Hysomide 20 mg Tablet is a loop diuretic used to treat edema (fluid retention) associated with congestive heart failure, liver cirrhosis, and kidney disease. It is also used to manage mild to moderate hypertension.
Uses & Indications
Dosage
Adults
Initial dose 20-40 mg once daily, adjusted based on response. Maintenance: 20-80 mg daily, as a single dose or divided doses.
Elderly
Lower initial doses (e.g., 20 mg) may be appropriate, with careful titration due to increased sensitivity and potential for adverse effects.
Renal_impairment
Dose adjustment may be necessary. Higher doses might be required in severe renal failure, but careful monitoring is crucial to avoid fluid and electrolyte imbalance.
How to Take
Take orally, preferably in the morning to avoid nocturnal diuresis. Can be taken with or without food. Follow your doctor's instructions.
Mechanism of Action
Hydrosemide, a loop diuretic, acts by inhibiting the Na-K-2Cl cotransporter in the thick ascending limb of the loop of Henle in the kidney. This prevents the reabsorption of sodium, potassium, and chloride ions, leading to increased excretion of water, sodium, chloride, and potassium. This reduces fluid volume and, consequently, blood pressure.

Side Effects
Contraindications
- •Hypersensitivity to Hydrosemide or sulfonamide derivatives
- •Anuria (absence of urine production)
- •Severe electrolyte depletion (e.g., severe hypokalemia, hyponatremia)
- •Hepatic coma and precoma associated with electrolyte imbalance
Drug Interactions
Digoxin
Increased risk of digoxin toxicity due to hypokalemia caused by Hydrosemide.
Corticosteroids
Increased risk of hypokalemia.
Other Antihypertensives
Potentiated hypotensive effect, leading to excessive blood pressure drop.
Aminoglycoside Antibiotics
Increased risk of ototoxicity (ear damage).
NSAIDs (Nonsteroidal Anti-Inflammatory Drugs)
May reduce the diuretic and antihypertensive effects of Hydrosemide and increase the risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dehydration, electrolyte imbalance (hypokalemia, hyponatremia, hypochloremia), hypotension, and cardiovascular collapse. Management involves symptomatic and supportive care, including fluid and electrolyte replacement.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Hydrosemide is excreted in breast milk; therefore, caution should be exercised or breastfeeding should be discontinued.
