Description
Overview of the medicine
Infanil 125 mg Suspension is an oral preparation of paracetamol, commonly used to relieve mild to moderate pain and reduce fever in infants and children. It works by inhibiting prostaglandin synthesis in the central nervous system.
Uses & Indications
Dosage
Adults
Not typically used for adults in 125 mg suspension. Adult dosage usually 500-1000 mg every 4-6 hours.
Elderly
Not typically used for elderly in 125 mg suspension. Elderly dosage should be carefully considered, similar to adults but with caution in renal/hepatic impairment.
Children
10-15 mg/kg body weight every 4-6 hours, not exceeding 4 doses in 24 hours. Max daily dose 60 mg/kg.
Renal_impairment
Dosage interval may need to be extended in severe renal impairment. Consult a physician.
How to Take
Administer orally, preferably with or after food to minimize gastrointestinal upset. Use a measuring spoon or oral syringe for accurate dosing.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system by inhibiting prostaglandin synthesis, which is responsible for its analgesic and antipyretic effects. It has a weaker peripheral effect on prostaglandin synthesis.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Mainly excreted in urine as glucuronide and sulfate conjugates.
Half life
1.5 to 3 hours in adults; slightly shorter in children.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation, with a small amount metabolized by cytochrome P450 enzymes to a toxic intermediate (NAPQI).
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment
Drug Interactions
Warfarin
May enhance the anticoagulant effect, increasing risk of bleeding.
Cholestyramine
May reduce paracetamol absorption if given within 1 hour.
Metoclopramide, Domperidone
May increase the rate of paracetamol absorption.
Carbamazepine, Phenytoin, Phenobarbital, Rifampin
May increase the risk of hepatotoxicity by inducing paracetamol metabolism.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose can lead to severe, potentially fatal, hepatic necrosis. Symptoms include nausea, vomiting, abdominal pain, and sweating. Management involves gastric lavage, activated charcoal, and administration of the antidote N-acetylcysteine.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at therapeutic doses, but should be used under medical advice.

- Renal function tests (in cases of chronic high-dose therapy)
Doctor Notes
- Educate parents/caregivers on proper dosing, especially for pediatric patients.
- Emphasize the risk of hepatotoxicity with overdose; ensure patients are aware of other paracetamol-containing products.
- Advise caution in patients with pre-existing liver conditions.
Patient Guidelines
- Do not exceed the recommended dose.
- Always use an accurate measuring device (spoon or syringe) for pediatric suspension.
- Do not use with other paracetamol-containing products.
- Consult a doctor if symptoms persist for more than 3 days (fever) or 5 days (pain).
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. If it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
