Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Typically 200 mg to 400 mg once daily or 200 mg twice daily for 7-14 days. For specific severe infections or as directed by physician, 500 mg once daily might be prescribed.
Elderly
No specific dosage adjustment required unless severe renal impairment is present.
Renal_impairment
Dose adjustment required for creatinine clearance < 60 mL/min. For creatinine clearance 21-60 mL/min, 75% of standard dose. For creatinine clearance < 20 mL/min, 50% of standard dose.
How to Take
Administer orally. Can be taken with or without food. For best absorption, it is often recommended to take with food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Onset of action typically within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine (approximately 50% of an absorbed dose) and via non-renal mechanisms (bile/feces).
Half life
Plasma half-life is approximately 3-4 hours.
Absorption
Oral bioavailability is 40-50%, reaching peak plasma concentrations in 2-6 hours.
Metabolism
Partially metabolized in the liver; however, most of the absorbed dose is excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions to penicillin (e.g., anaphylaxis).
Drug Interactions
Nifedipine
May increase cefixime's bioavailability.
Carbamazepine
Concomitant use may increase carbamazepine levels.
Oral Contraceptives
May reduce the effectiveness of oral contraceptives, requiring additional birth control methods.
Warfarin and other anticoagulants
May increase the anticoagulant effect, leading to increased risk of bleeding. Monitor INR/PT.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Supportive therapy should be administered.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Use with caution in nursing mothers as cefixime is excreted in breast milk in small amounts.
