Overview of the medicine
Ipsavent (Ipratropium Bromide) is an anticholinergic bronchodilator used to treat bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. It helps to relax the muscles in the airways, making breathing easier.
Uses & Indications
Dosage
Adults
MDI: 2 puffs (40 mcg) four times daily. Nebulizer: 0.5 mg (2.5 mL of 0.2 mg/mL solution) 3-4 times daily.
Elderly
No specific dosage adjustment needed, but caution in elderly with prostatic hyperplasia or glaucoma.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment.
How to Take
For inhalation only. Shake MDI well before use. Use with a nebulizer machine for nebulizer solution. Avoid spraying into eyes.
Mechanism of Action
Ipratropium Bromide is a non-selective muscarinic antagonist. It blocks muscarinic cholinergic receptors in the bronchial smooth muscle, leading to bronchodilation. It inhibits vagally-mediated reflexes by antagonizing the action of acetylcholine at the muscarinic receptors on bronchial smooth muscle.

Absorption
Minimally absorbed systemically after inhalation; most of the dose is deposited in the gastrointestinal tract and excreted in feces.

Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Ipratropium Bromide or atropine and its derivatives
- •Acute bronchospasm where rapid response is required
Drug Interactions
Beta-agonists
Can be used safely in combination for enhanced bronchodilation.
Other anticholinergics
May enhance anticholinergic effects.
Storage
Store at room temperature (15-30°C). Protect from freezing and direct sunlight. Keep out of reach of children.
Overdose
Symptoms: Dry mouth, blurred vision, tachycardia. Management: Primarily symptomatic and supportive treatment. Monitor vital signs.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excretion into breast milk is unknown, use with caution in nursing mothers.
