Overview of the medicine
Keteks (Telithromycin) is an antibiotic belonging to the ketolide class, primarily used to treat respiratory tract infections. It works by inhibiting bacterial protein synthesis. Due to potential serious side effects, its use is restricted to specific indications.
Uses & Indications
Dosage
Adults
800 mg once daily for 7-10 days depending on the infection.
Elderly
No dose adjustment required for elderly patients with normal renal function.
Renal_impairment
For severe renal impairment (CrCl < 30 mL/min), dose should be reduced to 600 mg once daily. For hemodialysis patients, administer after dialysis.
How to Take
Administer orally, with or without food.
Mechanism of Action
Telithromycin inhibits bacterial protein synthesis by binding to the 23S rRNA of the 50S ribosomal subunit. This prevents the translocation step and formation of the 50S ribosomal subunit. Its binding affinity is stronger than macrolides, especially in resistant strains.

Onset
Clinical effects typically observed within a few days.
Excretion
Excreted through feces (primarily) and urine. Approximately 7% excreted unchanged in urine.
Half life
Approximately 9-10 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1 hour.
Metabolism
Primarily metabolized in the liver via CYP3A4 and non-CYP pathways. Some active metabolites are formed.
Side Effects
Contraindications
- •Known hypersensitivity to telithromycin or any macrolide antibiotic
- •Patients with myasthenia gravis (due to risk of respiratory failure)
- •Patients with a history of hepatitis and/or jaundice associated with telithromycin
- •Concurrent use with colchicine in patients with renal or hepatic impairment
Drug Interactions
Digoxin
May increase digoxin levels, monitor closely.
Colchicine
Increased risk of colchicine toxicity, especially in renal/hepatic impairment.
Oral Contraceptives
No significant interaction, but advise backup method for severe diarrhea.
Statins (e.g., Simvastatin)
Increased risk of myopathy/rhabdomyolysis due to CYP3A4 inhibition.
CYP3A4 Inducers (e.g., Rifampin)
May decrease telithromycin plasma concentrations, reducing efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole)
May increase telithromycin plasma concentrations, increasing risk of adverse effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe gastrointestinal disturbances, hepatotoxicity, and visual disturbances. Treatment is supportive and symptomatic; no specific antidote exists. Gastric lavage and activated charcoal may be considered if recent ingestion.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Telithromycin is excreted in breast milk; caution should be exercised when administered to a nursing woman.
