Overview of the medicine
Kofen 0.25% Eye Drop is an ophthalmic solution containing Ketorolac Tromethamine, a non-steroidal anti-inflammatory drug (NSAID). It is used to relieve eye pain and inflammation following cataract surgery and to treat seasonal allergic conjunctivitis symptoms.
Uses & Indications
Dosage
Adults
1 drop in the affected eye(s) four times a day.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment required for renal impairment due to minimal systemic absorption.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, instill one drop, then gently close eye. Do not touch the dropper tip to any surface to avoid contamination. If using other eye drops, wait at least 5 minutes between applications.
Mechanism of Action
Ketorolac Tromethamine inhibits the cyclooxygenase (COX-1 and COX-2) enzymes, thereby blocking the synthesis of prostaglandins, which are mediators of inflammation, pain, and fever. In the eye, this reduces ocular inflammation and pain.

Excretion
Primarily renal (kidney) excretion if absorbed systemically.
Half life
Approximately 3.5-6 hours in systemic circulation, but less relevant for ophthalmic use due to minimal systemic absorption.

Minimal systemic absorption after ocular administration. Plasma concentrations are typically undetectable or very low.
