Description
Overview of the medicine
Kofen 1 mg Tablet contains Clonazepam, which belongs to the benzodiazepine class of drugs. It is primarily used to treat various types of anxiety disorders, panic disorders, and certain seizure conditions. It works by calming the brain and nerves.
Uses & Indications
Dosage
Adults
Initial dose 0.25-0.5 mg two to three times daily, adjusted gradually based on response, up to a maximum of 4 mg/day.
Elderly
Lower initial doses (e.g., 0.25 mg once daily) are recommended due to increased sensitivity and potential for adverse effects. Dose titration should be cautious.
Renal_impairment
No specific dosage adjustments are typically required, but caution is advised in severe impairment due to potential for accumulation of metabolites.
How to Take
Take Kofen 1 mg Tablet orally with or without food, usually two to three times daily as directed by your doctor. Swallow the tablet whole with water. Do not crush, chew, or break the tablet.
Mechanism of Action
Clonazepam enhances the activity of gamma-aminobutyric acid (GABA), a neurotransmitter that inhibits brain activity. By increasing GABA's effects, it produces sedative, anxiolytic, anticonvulsant, and muscle relaxant properties.
Pharmacokinetics
Onset
Within 30-60 minutes for sedative effects.
Excretion
Mainly excreted in the urine as metabolites, with a small amount excreted in feces.
Half life
18-50 hours (average 30-40 hours).
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by nitroreduction, to form inactive metabolites.
Side Effects
Contraindications
- •Acute narrow-angle glaucoma
- •Severe liver disease
- •Myasthenia gravis
- •Hypersensitivity to clonazepam or other benzodiazepines
Drug Interactions
CYP3A4 Inducers (e.g., rifampin, phenytoin, carbamazepine)
May decrease clonazepam plasma levels, potentially reducing its effectiveness.
CYP3A4 Inhibitors (e.g., ketoconazole, fluconazole, grapefruit juice)
May increase clonazepam plasma levels, leading to enhanced effects and side effects.
CNS Depressants (e.g., alcohol, opioids, barbiturates, antidepressants)
Increased risk of sedation, respiratory depression, and hypotension.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, ataxia, diminished reflexes, coma, and respiratory depression. Management involves supportive care, maintaining a patent airway, and monitoring vital signs. Flumazenil may be used as an antidote, but with caution due to the risk of seizures in benzodiazepine-dependent patients.
Pregnancy & Lactation
Pregnancy Category D: Clonazepam may cause fetal harm when administered to a pregnant woman. It is excreted in breast milk and can cause sedation and feeding difficulties in nursing infants. Use should be avoided during pregnancy and lactation unless the potential benefits outweigh the risks.
