Description
Overview of the medicine
Labegest 100 mg Tablet contains Labetalol, an alpha and beta-adrenergic blocking agent used to treat high blood pressure (hypertension). It works by relaxing blood vessels and slowing the heart rate, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initially 100 mg twice daily, may be increased in increments of 100 mg twice daily every 2-3 days until desired blood pressure response is achieved. Usual maintenance dose is 200-400 mg twice daily. Max dose: 2400 mg/day.
Elderly
Lower initial doses (e.g., 50 mg twice daily) may be appropriate, with gradual titration based on response and tolerability.
Renal_impairment
No specific dose adjustment is usually necessary for patients with renal impairment, but caution is advised in severe cases.
How to Take
Take orally, with or without food. To minimize orthostatic effects, it is generally recommended to take with food.
Mechanism of Action
Labetalol is a non-selective beta-adrenergic blocker and a selective alpha1-adrenergic blocker. It blocks beta1 and beta2 receptors, causing a decrease in heart rate and myocardial contractility, and blocks alpha1 receptors, leading to vasodilation and reduced peripheral vascular resistance. The combined effect lowers blood pressure.
Pharmacokinetics
Onset
Oral: 20 minutes to 2 hours; IV: 2-5 minutes.
Excretion
Primarily excreted in urine (approximately 55-60%) and feces (approximately 30%).
Half life
Oral: 6-8 hours; IV: 5-5.5 hours.
Absorption
Well absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via conjugation and glucuronidation.
Side Effects
Contraindications
- Bronchial asthma or chronic obstructive pulmonary disease (COPD)
- Decompensated heart failure
- Second or third-degree atrioventricular (AV) block
- Severe bradycardia
- Cardiogenic shock
- Hypersensitivity to Labetalol
Drug Interactions
Cimetidine
Increases Labetalol bioavailability.
Tricyclic Antidepressants (TCAs)
May increase the risk of tremor.
Nonsteroidal Anti-inflammatory Drugs (NSAIDs)
May reduce the antihypertensive effects of Labetalol.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and hypotension.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe hypotension, bradycardia, bronchospasm, and heart failure. Treatment involves symptomatic and supportive measures. Atropine for bradycardia, vasopressors for hypotension, and bronchodilators for bronchospasm.
Pregnancy & Lactation
Pregnancy Category C (old system); however, it is commonly used for hypertension in pregnancy due to its relatively good safety profile. Labetalol is excreted in breast milk; caution is advised during breastfeeding.
